南京市中小学生健康素养水平及影响因素分析

Health literacy and its influencing factors among primary and high school students in Nanjing city, China

      目的  了解江苏省南京地区中小学生健康素养水平及其影响因素,为制定针对性的健康素养干预策略和措施提供依据。
      方法  采用二阶段整群随机抽样方法,于2018年5 — 6月选择南京市4~12年级学生共4 401人进行问卷调查;采用单因素χ2检验和多因素logistic回归对其健康素养影响因素进行分析。
      结果  总健康素养水平为85.7 %,小学、初中和高中学生健康素养水平分别为73.7 %、90.3 % 和95.1 %。多因素logistic回归显示,女生健康素养水平高于男生(OR = 1.56,95 % CI = 1.30~1.87);与低年级学生和自评学习成绩较差学生相比,中年级学生(OR = 4.29,95 % CI = 3.45~5.33)、高年级学生(OR = 8.29,95 % CI = 6.24~11.01)和自评学习成绩一般(OR = 1.86,95 % CI = 1.48~2.34)或良好的学生(OR = 3.07,95 % CI = 2.42~3.89)健康素养水平较高;与父母文化程度较低学生相比,父母文化程度较高的学生健康素养水平较高(OR = 1.78,95 % CI = 1.42~2.24)。
      结论  南京市中小学生健康素养总体水平较高,但不同特征学生之间健康素养水平存在差异。性别、年级、自评学习成绩和父母文化程度是其健康素养的主要影响因素。

     

    Abstract:
      Objective  To examine the level and associates of health literacy (HL) among primary and high school students in Nanjing city of Jiangsu province and to provide evidences for developing targeted interventions on HL in the students.
      Methods  Using two-stage random cluster sampling, we recruited 4 498 students aged 9 – 17 years at 36 primary and high schools in all administrative districts of Nanjing municipality and conducted a self-administered questionnaire survey during May – June, 2018. Chi-square test and the multivariate logistic regression model were adopted to explore the influencing factors of HL.
      Results  Among the 4 401 students completing the survey, 85.7% were assessed with adequate HL and the proportions of the students with adequate HL were 73.7%, 90.3%, and 95.1% for the grade 4 – 6 pupils, junior high school students, and senior high school students, respectively. The results of multivariate logistic regression analysis demonstrated that the students with following characteristics were more likely to have adequate HL: girl students (versus boy students: odds ratio OR = 1.56, 95% confidence interval 95% CI: 1.30 – 1.87); upper grade students (OR = 4.29, 95% CI: 3.45 – 5.33 for junior high school vs. primary school and OR = 8.29, 95% CI: 6.24 – 11.01 for senior high school vs. primary school); reporting better self-rated academic performance (OR = 1.86, 95% CI: 1.48 – 2.34 for normal vs. poor and OR = 3.07, 95% CI: 2.42 – 3.89 for good vs. poor); and higher parental educational attainment (versus lower: OR = 1.78, 95% CI: 1.42 – 2.24).
      Conclusion  Among primary and high school students in Nanjing city, the health literary is at a relatively high level and mainly influenced by gender, schooling grade, self-rated academic performance, and parental educational attainment.
