危重症监护室呼吸机相关性肺炎病原菌研究

Study on pathogens of pneumonia correlated patients with mechanical ventilation in intensive care unit

  • 摘要:
      目的   对呼吸机相关性肺炎病原学构成及耐药情况, 为临床治疗提供依据。
      方法   对我院重症监护(ICU)室收住的86例机械通气患者, 共中64例发生肺部感染的致病菌进行培养, 并对致病菌进行药敏试验。
      结果   检出致病菌84例, 其中革兰阴性细菌(G-菌)占69.05%(58株), 革兰阳性细菌(G+菌)占19.05%(16株), 真菌占11.90%(10株); 前4位致病菌分别为铜绿假单胞菌(29株)、肺炎克雷白氏菌(14株)、白色念球菌(10株)、金黄色葡萄球菌(9株)。药敏结果显示这类菌株药现象有严重。
      结论   呼吸机相关性肺炎病原菌以G-菌为主, 呈多重耐药, 临床上合理使用抗菌素至关重要。

     

    Abstract:
      Objective   To analyze the constitute of the pathogens of pneumonia in patients by mechanical ventialtion and the results of drugresistane.
      Methods   The pathogens and the drug sensitivity of 64 cases with v entilator-associated-pneumonia in 86 cases in ICU were detected.
      Results   84 strains pathogens were isolated by bacterial culture, the ratio of the gram-negative bacteria to total bacteria isoalted was 69.05%(58 strains), and the gram-posit ive bacteria was 19.05%(16 strains), the fungi was 11.90%(9 strains).P.aerug inose(29 strains), P.klebsiella(14 strain), moniliasis(10 strains)and S.aureus(10 strains)were the four common kinds of pathogens, the multi-drug-resistance on antibiotics was serious.
      Conclusion   Gram-negative bacteria were the more common pathogens of pneumonia isolated from patients with mechanical ventilation in ICU, the pathogens were multi-drug-resistance to antibiotics, and reasonable using antibiotics were extremely important.
