Comparison of cerebrovascular hemodynamic indexes before and after intervention

      目的   比较和分析干预前后脑卒中高危人群的脑血管血液动力学指标(CVHI)的变化, 为卒中的预防提供参考。
      方法   以整群抽样的方法抽取上海市南汇区三墩、万祥、书院3镇的35岁以上人群36972例, 筛选出具有高血压、糖尿病、心脏病、卒中家族史、高血压家族史等危险因素的脑卒中易感人群6170人为目标。基线调查时进行CVHI检测, CVHI积分值70分以下的高危个体分别给予一般干预(一般干预组)和在一般干预基础上加脑安胶囊进行重点干预(重点干预组)。对干预前后均有CVHI检测结果且积分值在70分以下的999例干预对象进行干预前后CVHI变化的比较分析。
      结果   一般干预组干预前后的CVHI平均积分值分别为49.7±18.8和73.4±26.6, 干预后积分升高47.7%;重点干预组干预前后的CVHI平均积分分别为41, 3±19.8和71.1±26.1, 干预后积分升高72.2%。各干预组CVHI单项指标中脑血管最小血流速度(Vmin), 最小血流速度(Vmax)在干预后均显著升高; 脉搏波波速(WV)、特性阻抗(Zcv)、外周阻力(Rv)及动态阻力(DR)干预后均显著降低。重点干预组多数指标的变化幅度大于一般干预组。
      结论   干预后CVHI总积分增高、各单项指标均有显著改善, 重点干预组效果优于一般干预组。干预措施能使脑血管功能明显改善。

     

    Abstract:
      Objective   To analyse changes of cerebrovascular hemody mamic indexes (CVHI) in high risk population of stroke before and after intervention and provide reference for stroke primary prevention.
      Methods   Target populations over 35 years were selected by cluster sampling from Sandun, Wangxiang and Shuyan Town in Nanhui County of Shanghai.6 170 persons with exposure of risk factor of stroke, such as hypertension, diabetes, heart disease, family history of stroke or hypertension, were selected from 36972 target populations. Examination of CVHI were carried out during baseline investigation and 999 persons whose CVHI score below 70 points were divided into two groups according to personal wish. One group received Chinese medicine named Nao an Capsule based on general intervention, and the other only received general intervention. CVHI score and single indexes were compared before and after three-year intervention.
      Results   CVHI score before and after general intervention were 49.7±18.8 and 73.4±26.6, and improved by 47.7 percent. The score before and after Naoan Capsule intervention were 41.3±19.8 and 71.1±26.1, and improved by 72.2 percent. CVHI, such as Vmin, Vmax, Vmean were significantly increased after intervention in both groups. However, Rv, WV, Zcv, DR and CP were obviously decreased. Range of CVHI in Naoan capsule intervention group were more significant than that in general intervention group.
      Conclusion   CVHI score as well as single index were significantly improved after intervention and the Naoan capsule intervention was more effective. The intervention measures could obviously improve cerebral vascular function.
