吡喹酮对疑似脑囊虫病患者诊疗作用的研究

摘要:
目的观察吡喹酮(PQT)对疑似脑囊虫病患者的诊疗作用。
方法应用大剂量PQT〔50mg/(kg°d)〕12d对疑似脑囊虫病患者进行了诊断性治疗。
结果 93例疑似脑囊虫病人有38例确诊患脑囊虫病, 42例基本确诊患其它中枢神经系统疾病, 13例仍诊断不明。38例脑囊虫病患者有27例服吡喹酮后出现杀虫反应, 其中疗后有17例患者复查脑CT或MRI结果显示病灶缩小、周围水肿吸收或消失, 有2例患者复查脑CT显示病灶增多, 且呈囊性影像表现。另11例确诊为脑囊虫病的患者服药期间未出现杀虫反应, 根据疗后复查CT或MRI, 病灶较疗前吸收或消失而确诊。确诊的38例脑囊虫病患者再经过1~2个疗程抗囊治疗后临床症状消失, 其中有31例患者复查脑CT显示病灶完全吸收者26例, 病灶出现钙化者5例。
结论大剂量吡喹酮对疑似脑囊虫病患者具有较好的诊疗作用。

Abstract:
Objective To observe the diagnosis and therapy effect of praziquantel(PQT)on suspected cerebral cysticercosis.
Methods To apply diagnosis therapy on suspected cerebral cysticercosis with large dose PQT(50mg/kg/d, 12d).
Results Thirtyeight cases were confirmed cerebral cysticercosis in 93 cases of suspected cerebral cysticercosis, 42 cases were basical diagnosed other central nervous system diseases, 13 cases were still unconfirmed.27 of 38 cases of cerebral cysticercosis showed obvious insecticide effect, 17 cases of which counterchecked CT or MRI showed the foci shrinked and surrounding edema obsorbed or disappeared, 2 cases showed the foci manifold and had cyst appearance.Other 11 cases of cerebral cysticercosis did not show obvious insecticide effect, they were diagnosed by CT or MRI appearance of focus shrinked or disappeared.The clinical symptom of 38 cases of cerebral cysticercosis disappeared after 1 or 2 period of anti-cysticercosis treatment, 31 cases of which countercheck CT or MRI showed the foci absorbed absolutely, 5 cases had calcify appearance.
Conclusion Large dose of PQT had preferable diagnosis and treating effect on suspected cerebral cysticercosis.
