中国肾综合征出血热监测研究

Study and surveillance of HFRS( Hemorrhage Fever with Renal Syndrome) in China

  • 摘要: 自1984年以来,在我国30个省(自治区?直辖市)由卫生部和中国预防医学科学院先后建立了48个全国肾综合征出血热(HFRS)监测点(自1991年调整为41个,即调下的有延边州?徐州市?镇江市?沛县?巩义市?平顶山市?成都市,调上的有蛟河市?大丰县?东海县?确山县?淮阳县?涪陵市)。并委托我所负责具体业务和有关监测研究工作。14年来,通过HFRS监测点多年动态的人间疫情监测?宿主动物监测?疫源地和疫区监测和控制效果监测,以及自然科学基金课题(疫源地结构和传播机理研究)等内容的监测研究,并结合对全国范围的调查和实验室研究,取得了大量的科学数据。1984~1997年共检测血清203375人份。查证临床诊断病例血清学符合率,1984~1990年为80.37%(9153/11389);1991~1997年为72.44%(11512/15892)。1984~1990年疑似病例血清学符合率为46.51%(10942/23527);健康人群HV隐性感染率为2.76%(3838/139063);390例HFRS恢复期病人血清学分型结果,姬鼠型占53.30%,家鼠型占41.77%,未定型占4.93%。1984~1990年检测HFRS病人双份血清3144人份,发病6天内IgG抗体滴度≤1∶320的占84.83%,恢复期血清滴度≥1∶320的占82.06%。患HFRS病后不同时间检则IgG抗体存在时间,结果短者1个月,长者达28年。1991~1997年疫情漏报率波动在0.70%~9.24%之间,平均为2.87%(1031/36295)。累计放鼠夹4633647夹次,捕获小兽395453只,平均密度为8.53%;检测小兽汉坦病毒(HV)抗原356115只,阳性9797只,小兽带病毒率为2.75%。不仅摸清了我国HFRS在疫区人群和疫源动物中的流行本底情况,而且在HFRS人间疫情(流行规律、流行特征)、宿主动物(种类组成、主要传染源、传播机理)、疫源地和疫区分布特征、控制效果和疫情预测等诸方面,均取得了一系列的科技成果。

     

    Abstract: 48 Surveillance sopts of HFRS have been established in 30 provinces(or Autonomous regions and Municipal Cities)since 1984 by the Ministry of Public Health and Chinese Academy of Preventive Medicine.In 1991 these surveillance spots were adjusted to 41:Yanbian District Xuzhou City,Zhenjiang City Pei County Gongyi City,Pingdingshan City,and Chengdu City were canceled and Jiao he City,Dafeng County,Donghai County,Queshan County,Huaiyang County and Fuling City were added Our Institute was entrusted for in charge of the professional work and study of all these surveillance spots.During the 14 years we had the surveillance andstudy of HFRS trends on human epidemic situation host animals epidemic sources and areas effect control as well as the program of NSFC General Project(study on the structure of epidemic sources and areas and the spreading mechanisms) combined with the whole nation as investigation and laboratory research we got a large number of scientific data.From 1984 to 1997 we examined 203375 samples of human serum.Accordance of them with clinical diagnosis were as follows 80.37% (9153/11389) from 1984 to 1990 and 72 44% ( 11512/15892) from 1991 to 1997.From 1984 to 1990 we examined the serum samples of suspicious patients the accordance of them with clinical diagnosis was 46.51%( 10942/23527 ) the rate of recessive infection of HV in health pecpfe was 2.76% (3838/139063) the classification result of 390 serum samples from recovered patients showed that the virus type of Apodemus agrarius was 53.30%.the type of domestic mouse was 41.77% and the undefined type was 4.93%.Form 1984 to 1990 we also examined the double serum amples of 3144 patients 84 83% of them had 1:320 IgG in 6th days of HFRS and 82 06% of them had 1:320 titer of IgG during the recovered period.We examined the duration time f IgG after suffering from HFRS and the results showed that the duration time of IgG was from one month to 28 years.In 1991 to 1997 the rate of failing to report was fluctuated from 0.70% to 9.24% and the average of it was 2.87% (1031/36295).The total number of mouse clips that had been distributed were 4633647; the number of the small animals that were captured was 395453 arid the average density of them was 8.53%; the number of small animals that were examined the antigen of HV was 356115 arid the positive number of them was 9797; the rate of HV carrier in the small animals was 2.75%.From this surveillance and study we not only understood the epidemic situation of HFRS in human and animals of epidemic sources in epidemic areas but also got a series of scientific achievements on epidemic situation of human (epidemic regulation and character) host animals (classification composition main epidemic sources and spreading mechanisms) district characters of the epidemic sources and features of the epidemic areas,effect control and epidemic forecasting.
