云南省2017年人群麻疹抗体水平和易感性分析

Measles antibody and susceptibility among general populations in Yunnan province in 2017

    目的 分析云南省健康人群麻疹抗体检测结果,找出易感人群、薄弱地区,为云南省麻疹防控提供科学依据。
    方法 采取多阶段随机抽样的方法,在2017年对云南省21个县63个监测点5 292人进行问卷调查及采集血标本,应用酶联免疫吸附试验(ELISA)检测麻疹抗体,检测结果录入数据库,利用SPSS17.0软件进行统计分析。
    结果 麻疹抗体阳性率为90.91 %(95 % CI = 90.14~91.68 %),抗体几何平均浓度为967.28 mIU/mL。不同地区、不同年龄、不同民族、不同免疫史麻疹抗体阳性率及几何平均浓度差异均具有统计学意义。23~30岁人群抗体水平显著低于其他年龄段,易感性最高(OR = 2.52,95 % CI = 1.75~3.65)。其他民族抗体水平低于汉族(t = 3.21,P < 0.01)。免疫剂次越多,抗体水平越高,但随免疫次数增多抗体水平增加幅度会减少。
    结论 云南省人群麻疹免疫力暂不能形成较好免疫屏障,阻断麻疹病毒传播。切实落实基础免疫预防接种工作并适时开展补充免疫能巩固免疫屏障。

     

    Abstract:
    Objective To analyze measles antibody level in healthy populations and to identify susceptible populations in Yunnan province for providing evidences to prevention and control of measles.
    Methods We selected 5 292 residents at 63 disease surveillance sites in 21 counties of Yunnan province with multistage random sampling and conducted a questionnaire survey and collected blood samples for detection of measles antibody with enzyme-linked immunosorbent assay (ELISA). We adopted SPSS 17.0 software in data analysis.
    Results The positive rate of measles antibody was 90.91% (95% confidence interval 95% CI: 90.14 – 91.68%) and the geometric mean of antibody concentration was 967.28 mIU/ml. Both the positive rate and geometric mean of measles antibody differed significantly by region, age, nationality, immunization history among the participants. The participants at ages of 23 – 30 years had a significantly lower measles antibody levels and the highest susceptibility to measles virus (odds ratio OR = 2.52, 95% CI: 1.75 – 3.65) compared to those at other ages. The Han participants had a higher measles antibody level than the participants of other nationalities. The measles antibody level increased with the times of vaccination reported by the participants but the increment did not increase with the times of vaccination accordingly.
    Conclusion The measles immunization level is not high enough to form an efficient immune barrier for measles epidemic among residents in Yunnan province and the result suggests that effective basic vaccination and timely supplemental immunization need to be implemented in the population.
