从妇女保健看京沪健康战略对社会环境支撑程度改善

Improvement effect of health strategy on social environment support to maternal health care in Beijing and Shanghai

      目的  评价京沪两地健康战略发布前后,社会环境对妇保体系支撑程度的改善情况,论证完善健康战略配套措施并保障其落实的重要性。
      方法  于2016 — 2017年,系统收集两地所有妇保相关的政策文件(北京共纳入232份,上海共纳入417份文件),量化计算并比较健康战略发布前后政策环境决定程度、法律保障程度、经济支撑程度、文化引领程度4个方面,及其社会环境支撑程度的变化状况。
      结果  健康战略发布以后,京沪两地社会环境均有较大幅度的提升,北京从29.5 % 上升至52.6 %,上海从26.4 % 上升至46.1 %。但其包含的四个指标仍然有很大的上升空间,尤其是与健康战略直接相关的政策环境决定程度(京沪分别上升至30.3 % 和29.1 %)和经济发展支撑程度(京沪分别上升至54.8 % 和42.1 %)。
      结论  健康优先战略的发布对提升社会环境的支撑程度至关重要。后续亟需形成可操作的政策并落实到位,推进健康战略在各方落实职责分工、社会经济的支撑、各方协同支持公共卫生氛围的形成中发挥更好的引领作用。

     

    Abstract:
      Objective  To evaluate the improvement of the social environment support for the maternal health care system before and after the initiation of the health strategy in Beijing and Shanghai, and to verify the significance of implementing the strategy.
      Methods  Totally 649 policy documents on maternal health care issued by administrative agencies of Beijing (232 documents) and Shanghai (417 documents) till 2017 were systematically retrieved. The quantitative indicators for changes in social environment support (including political environment determinant, legal guarantee, economic support, and cultural guidance) to maternal health care performance in the two municipalities before and after the initiation of the health strategy were calculated and compared.
      Results  After the initiation of the health strategy, the indicators for social environment support to maternal health care performance was increased substantially for both the municipalities (from 29.5% to 52.6% for Beijing and 26.4% to 46.1% for Shanghai); however, there were still potential improvements in the domain indicators for the four dimensions of the social environment support, especially for those of political environment determinant (increased to 30.3% and 29.1% in Beijing and Shanghai) and economic support (increased to 54.8% and 42.1% in Beijing and Shanghai).
      Conclusion  The initiation of the health priority strategy is critical to improving social environment support for the maternal health care performance and efficient policies related to the strategy needs to be developed and implemented urgently.
