2017 — 2018年上海市浦东新区13价肺炎链球菌多糖结合疫苗接种状况及安全性评价

Evaluations on coverage and safety of 13-valent pneumococcal conjugate vaccine inoculation in Pudong New Area of Shanghai, 2017 – 2018

      目的  通过对2017 — 2018上海市浦东新区13价肺炎链球菌多糖结合疫苗接种状况及安全性评价进行分析,为其在儿童中的接种提供相关依据。
      方法  选取2017 — 2018上海市免疫规划信息系统接种13价肺炎链球菌多糖结合疫苗的信息和中国免疫规划信息管理系统疑似预防接种不良反应监测信息,采用描述性流行病学方法对疫苗接种情况及AEFI报告发生率、三间分布等指标进行分析。
      结果  2017 — 2018年浦东新区13价肺炎链球菌多糖结合疫苗累积接种剂次48 712针次,发生AEFI病例355例,年平均报告发生率为666.76/10万剂。其中一般反应353例(99.44 %),异常反应2例(0.56 %)。AEFI的发生在男女性别中差异有统计学意义(χ2 = 4.575,P = 0.032),不同剂次的报告发生率差异有统计学意义(χ2 = 11.258,P = 0.010)。按接种至发病的时间间隔分析,353例一般反应中,≤ 1 d发生的285例,占80.74 %,2~3 d的66例,占18.70 %,≥ 4 d的2例,占0.56 %。2例异常反应中,分别发生在 ≤ 1 d和2~3 d各占50.00 %。
      结论  13价肺炎链球菌多糖结合疫苗AEFI报告发生率明显高于全区AEFI报告发生率,PCV13是蛋白结合疫苗,对机体而言是异体蛋白,可至机体产生接种反应,因此AEFI的发生率相对较高。随着国内专家学者对肺炎链球菌性疾病的关注度逐渐上升,PCV13也逐渐为更多的家庭所熟知,也是预防肺炎链球菌性疾病最直接有效的预防措施。

     

    Abstract:
      Objective  To examine vaccination status of 13-valent pneumococcal conjugate vaccine (PCV13) and to evaluate the safety of PCV13 inoculation among children in Pudong New Area of Shanghai from 2017 to 2018 for providing evidences to implementing PCV13 vaccination in children.
      Methods  We collected two sets of data relevant to PCV13 vaccination among children in Pudong New Area of Shanghai during 2017 – 2018, including information on completed inoculations from Shanghai Planned Immunization Information System and on suspected adverse events following immunization (AEFI) registered in National Planned Immunization Information Management System. Descriptive epidemiology method was used in analyses on PCV13 vaccination coverage and incidence and spatial-temporal distribution of suspected AEFI.
      Results  During the period, totally 48 712 doses of PCV13 were vaccinated and 355 AEFI were registered, with a reported average annual AEFI incidence rate of 666.76/100 000 doses. Of all the reported AEFI, 99.44% (n = 353) were general reactions and only 0.56% (n = 2) were abnormal reaction. The reported incidence rate of AEFI differed significantly by gender (χ2 = 4.575, P = 0.032) and dose order (χ2 = 11.258, P = 0.010). Among all reported general reactions, 285 (80.74%) occurred during the day, 66 (18.70%) after 2 – 3 days, and 2 (0.56%) after 4 or more days of the PCV13 inoculations, respectively; while, one of the two abnormal reactions occurred during the day and the other after 2 – 3 days of the inoculations.
      Conclusion  The reported incidence of PCV13-related AEFI was significantly higher than the overall AEFI incidence among the children Pudong New Area of Shanghai during 2017 – 2018. The high incidence may be associated with the protein-binding characteristics of PCV13.
