Distribution and antibiotic resistance of Gram-negative non-fermentative bacteria isolated in hospital environment

      目的  为了解医院环境中革兰阴性非发酵菌的分布状况及耐药情况。
      方法  2017年10月集中采集黑龙江省内11所三级甲等医疗机构不同重点科室环境物体表面及医护人员手样品198份,其中物体表面176份,医护人员手消毒后采样22份。使用法国梅里埃Vitek微生物鉴定仪对分离菌株进行鉴定及药敏试验。使用K-B法药敏纸片法对检出菌株进行同步耐药性检测。
      结果  22份医护人员手样品中未检出革兰阴性非发酵菌,物体表面检出15株,检出率为8.52%(15/176);以鞘氨醇单胞菌,鲍曼不动杆菌,栖稻假单胞菌为主,且均呈现多重耐药性;硝基呋喃类、叶酸代谢途径抑制剂类抗生素耐药率最高,达到80 %。
      结论  环境中革兰阴性非发酵菌耐药明显,应在临床治疗及选择抗生素时予以关注。

     

    Abstract:
      Objective  To examine the distribution and antibiotic resistance of Gram-negative non-fermentative bacteria isolated in medical environment.
      Methods  We collected 176 public facility and medical equipment surface swab samples and 22 swab samples of medical staff's hands after disinfection in 11 grade A tertiary hospitals in Heilongjiang province during October 2017. The identification and drug sensitivity test of the isolated bacterial strains were performed using the French MERIE Vitek microorganism identification instrument. Kirby-Bauer disk diffusion method was used in antibiotic resistance test for the isolated strains.
      Results  The total detection rate of Gram-negative non-fermentative bacteria was 7.58% for the facility and equipment surface swab samples. The main isolated bacteria were Sphingomonas paucimobilis, Acinetobacter baumannii, and Pseudomonas oryzihabitans; all the isolated strains were multiple antibiotic resistant and the highest antibiotic resistance rate (80%) was detected in the stains resistant to nitrofurans and folic acid metabolic pathway inhibitors. No Gram-negative non-fermentative bacteria positive samples was detected for the swabs of medical staff's hands.
      Conclusion  Antibiotic resistant Gram-negative non-fermentative bacteria are prevalent in medical service environment and the situation should be concerned in antibiotic selection in clinical practice.
