宁夏5县农村老年人生命质量及影响因素分析

Quality of life and its influencing factors among rural elderly in five counties of Ningxia Hui Autonomous Region

      目的  了解宁夏5县农村老年人生命质量状况,并探讨其影响因素,为改善农村老年人生命质量提供参考依据。
      方法  选取“农村居民家庭卫生健康询问调查”2015年随访数据,采用欧洲5维健康量表(EQ-5D量表)的方法对宁夏5县 2 595名农村老年人的生命质量及其影响因素进行分析。
      结果  调查对象中,男性1 379人(占53.1 %),女性1 216人(占46.9 %),平均年龄为(67.85 ± 6.33)岁。5维度测量中,存在问题维度是疼痛/不适占48.3 %,行动占35.2 %,日常活动占33.0 %,焦虑/抑郁占26.9 %,自我照顾占21.8 %;老年人自评健康得分(VAS)均值为(62.08 ± 17.46)分。在单因素分析中,性别、年龄、文化程度、婚姻状况、慢性病对生命质量各维度和VAS得分均有影响(P < 0.05);在多因素分析中,性别、年龄、慢性病对生命质量各维度和VAS得分均有影响(P < 0.05)。
      结论  宁夏5县农村老年人的生命质量较差,慢性病是主要影响生命质量的因素,政府应积极采取相应措施以提高老年人的生命质量。

     

    Abstract:
      Objective  To examine the quality of life (QOL) and its influencing factors among rural elderly in five counties of Ningxia Hui Autonomous Region (Ningxia), and to provide references for improving the QOL of rural elderly.
      Methods  We extracted information on 2 595 rural residents aged 60 years and above in five counties of Ningxia from the follow-up data of the Household Survey on Health Among Rural Residents in 2015. The Euro-Quality of Life-5 Dimension (EQ-5D) was adopted to assess the QOL of the participants and influencing factors of QOL were analyzed.
      Results  Of the participants averagely aged 67.85 ± 6.33 years, 1 379 (53.1%) were male and 1 216 (46.9%) were female. Among the participants, 48.3%, 35.2%, 33.0%, 26.9%, and 21.8% reported having problems in pain/discomfort, mobility, usual activities, anxiety/depression, and self-care, respectively; the self reported average visual analog score (VAS) for general health condition was 62.08 ± 17.46. The QOL and VAS score of the participants were significantly influenced by gender, age and chronic disease status based on both univariate and multivariate analysis (all P < 0.05); while education and marital status were significant impact factors only in univariate analysis (P < 0.05 for all).
      Conclusion  The quality of life is relatively poor and mainly influenced by chronic disease status among rural elderly in five counties of Ningxia; the results suggest that countermeasures are needed to promote the quality of life in the population.
