糖尿病及高危人群血清游离脂肪酸成分分析

Analysis on serum free fatty acid composition in patients with type 2 diabetes mellitus

      目的   研究2型糖尿病及高危人群血游离脂肪酸成分水平及其与血糖参数间关系.
      方法   用气相色谱/质谱(GC/MS)联用技术测定162例2型糖尿病及高危人群与43名正常对照组血游离脂肪酸成分水平.
      结果   糖尿病组不饱和脂肪酸(C18:2, C18:1, C20:4, C22:6, C20:3)高于正常对照组, 在C18:2, C20:3的差异有统计学意义(P < 0.01~P < 0.05);不饱和脂肪酸/总脂肪酸(UFA/TFA)、多不饱和脂肪酸/总脂肪酸(PUFA/TFA)、多不饱和脂肪酸/饱和脂肪酸(PUFA/SFA)显著高于正常对照(P < 0.01);饱和脂肪酸(C16:0)和SFA/TFA低于正常对照组, 差异有统计学意义(P < 0.01);亚油酸(C18:2)、n6/n3显著高于高危人群组(P < 0.05~P < 0.01).高危人群组UFA/TFA、PUFA/SFA显著高于正常对照组(P < 0.05).在研究对象中, 血糖与UFA/TFA、PUFA/TFA、PUFA/SFA、n6/n3呈正相关(r=0.190, 0.172, 0.190, 0.182, P < 0.01~P < 0.05), 血糖与SFA/TFA呈负相关(r=-0.190, P < 0.01).
      结论   2型糖尿病及高危人群中存在游离脂肪酸代谢紊乱.n6系多不饱和脂肪酸升高, 可能在糖尿病发病中起重要作用.调整膳食结构, 可以减少脂质过氧化损伤.

     

    Abstract:
      Objective   To explore the relationship between serum free fatty acid(FFA)composition and fasting blood glucose(FBG)in patients with type 2 diabetes mellitus and risk factors associated with diabetes(RFD).
      Methods   The serum FFAprofile was measured with the gas chromatography and mass spectrometer(GC/MS)in 162 patients and 43 control subjects.
      Results   The patients with diabetes had higherparameters of unsaturated fatty acid(UFA, C18:2, C18:1, C20:4, C22:6, C20:3), and polyunsaturated fatty acid(PUFA, C18:2, C20:3), UFA/TFA, PUFA/TFAand PUFA/SFAshowed a significantincrease as compared with those in the normal subjects(P < 0.01~P < 0.05).Saturated fatty acid(SFA, C16:0)and SFA/TFAin patients with diabetes were decreased remarkably than thatin the healthy controls(P < 0.01), theirlinoleic acid(C18:2)and n6/n3 were significantly increased compared to the patients with RFD(P < 0.05~P < 0.01).Subjects with RFDhad higherratio of UFA/TFA, PUFA/SFAcompared to the healthy controls(P < 0.05).FBGhad remarkably positive correlation with UFA/TFA, PUFA/TFA, PUFA/SFAand n6/n3(r=0.190, 0.172, 0.190, 0.182, P < 0.01~P < 0.05, respectively)and a negative correlation with SFA/TFA(r=-0.190, P < 0.01)in these all subjects.
      Conclusion   There is the FFAmetabolic abnormality in these patients.Increased levels of n6PUFAmay play a role in diabetic pathogenesis.Balanced dietary may improve upon the increased lipid peroxidation.
