目的观察幽门螺杆菌(Hp)根除治疗前后患者血清胃蛋白酶原(PG)含量的变化.
方法对287例Hp相关性胃病患者给予常规的奥美拉唑、阿莫西林、克拉霉素三联治疗, 除菌后6周检测其胃粘膜Hp、血清HpIgG抗体和Hp-DNA(PCR法)3项指标, 联合判断Hp根除效果; 分别于除菌前和除菌后3, 6周利用免疫量度放射分析法(IRMA)测定血清蛋白酶原含量, 比较Hp根除成功者(除菌成功组)和失败者(除菌失败组)血清蛋白酶原含量变化.
结果除菌成功103例, Hp根除率为35.89%.与除菌前比较, 除菌成功组的血清PGI、PGII含量明显降低, Ⅰ的比值显著升高.除菌失败组的血清PGI、PGII含量变化不明显, PGI/II的比值变化亦不显著.
结论根除Hp治疗前后血清PG发生特异性改变, 血清PG含量可作为Hp根除的判断指标.

Abstract:
Objective To investigate the variation of serum pepsinogen(PG)in patients with Helicobacter pylori(Hp) infection before and after the eradication.
Methods Three methods was used to determine the result of eradication 6 weeks after treatment in 287 patients with Hp infection, who were treated for eradication by routine triple therapy.: Hp-DNAPCR, HEstaining and ELISAanalysis.Serum PGconcentrations were detected with immunoradiometric assay(IRMA)in patients with Hp infection before and 3, 6 weeks after treatment.
Results Compared with serum level before treatment, serum PGIand PGIIconcentrations decreased in the eradication group, and PGI/IIratio also increased significantly.There were no remarkable changes with serum PGconcentrations in uneradication group.
Conclusion There are specific changes of serum PGconcentrations before and after the eradication.Serum PGconcentrations can be used as the index of Hp eradication.
