Expression of P-glycoprotein and its influencing factors in hepatocellular carcinoma

  • 摘要: 目的 探讨原发性肝细胞癌(HCC)中多药耐药基因P-糖蛋白(P-gp)的表达情况及其影响因素。方法 采用免疫组织化学(S-P)法检测135例HCC患者组织中P-gp的表达情况。结果 P-gp表达阳性率为80.74%;HCC患者P-gp表达阳性率在不同年龄、肿瘤直径大小间差异有统计学意义(P<0.05);低分化HCC患者中P-gp表达阳性率高于中、高分化HCC患者(P<0.05);相关分析结果显示,肿瘤细胞分化程度越低,P-gp表达阳性率越高(r=0.25,P=0.003);血清甲胎蛋白(AFP)升高患者P-gp表达阳性率高于血清AFP正常患者(P<0.01);谷草转氨酶(AST)升高患者P-gp表达阳性率高于AST正常患者(P<0.01);多因素Logistic回归分析结果显示,细胞分化程度低、血清AFP水平升高和AST水平升高是P-gp表达阳性的危险因素。结论 多药耐药基因P-gp表达与HCC患者细胞分化程度、血清AFP水平和AST水平有关。

     

    Abstract: Objective To investigate the expression of P-glycoprotein(P-gp) and its influencing factors in hepatocellular carcinoma(HCC).Methods The expression of P-gp was detected with S-P immunohistochemical technique in 135 cases of HCC.Results The positive rate of P-gp was 80.74%(109/135).The expression of P-gp was related to age and tumor size(P<0.05).The positive rate of P-gp in poorly-differenciated HCC was higher than that of in middle-differenciated or well-differenciated HCC(P=0.010).Spearman rank correlation test showed a correlation between the positive rate of P-gp and tumor differentiation(P=0.003).The expression of P-gp was higher in cases with elevated alpha fetoprotein(AFP) than that of with normal AFP(P=0.001).The expression of P-gp was higher in cases with elevated aspartate aminotrans ferase(AST) than that of with normal AST(P=0.005).Multiple variant regression analysis showed that there were correlations between the expression of P-gp and factors such as tumor poorly-differentiation,elevated AFP and elevated AST.Conclusion The expression of P-gp is related to tumor differentiation,AFP,and AST.
