摘要: 目的了解天津市参加城镇职工基本医疗保险的卵巢癌患者住院费用构成及其影响因素,为控制医疗费用的不合理增长提供参考依据。方法从天津市2003-2007年参保住院患者资料库中抽取卵巢癌患者1 635例,对其住院费用情况进行回顾性分析。结果天津市2003-2007年参保卵巢癌患者住院总费用为20 035 629.53元,人均住院费用中位数为9 824.59元;卵巢癌患者住院费用中药品费、检查费、治疗费、手术费、床位费、材料费、输血费、成分输血费和其他费用分别占72.35%、13.53%、5.88%、3.37%、3.22%、0.35%、0.39%、0.48%和0.43%。多元线性回归分析结果表明,药品费和检查费比重越大、住院天数越多、医院级别越高,参保卵巢癌患者住院费用越高。结论住院天数、医院级别、药品费比重和检查费比重是参保卵巢癌患者住院费用的主要影响因素。

Abstract: Objective To identify hospita lization expense and its influencing facyors among ovarian cancer patients with medical in surance in Tianjin for the control of irrational increase in medical expense.Methods Atotal of 1 635 ovarian cancer patients in a hospital of Tianjin from 2003 to 2007 were included and their hospitalization expenses were analyzed.Results The sum and the median of hospitalization cost perpatient was 20 035 629.53 RMB Yuan and 9824.59 RMB Yuan from 2003 to 2007.The items of the hospita lization cost for the patients were drug (72.35%), ex am ina tion (13.53%), treamtent (5.88%), operation (3.37%), bed charge (3.22%), materials (0.35%), blood transfusion (0.39%), component blood transfusion(0.48%), and other items(0.43%).The total cost was positively correlated with the duration of hospitalization and the grade of the hospital.Conclusion Reducing the proportion of drug and examination fee and reducing the duration of hospitalization are the key points to the control of ho spitalization expense for ovarian cancer patient.
