辽宁省医务人员工作倦怠状况分析

Assessment of job burnout among medical staff in Liaoning province

  • 摘要: 目的 了解辽宁省城乡各级医务人员工作倦怠水平及重度工作倦怠影响因素,为开展相关干预措施提供参考。方法 采用多阶段分层随机抽样方法,在辽宁省抽取3 876名医务人员(医生1 807人、护士1 405人、疾控人员664人)进行工作倦怠状况问卷调查。结果 辽宁省医务人员情绪衰竭、消极怠慢和成就感得分分别为(11.1±6.8)、(5.5±5.3)、(23.0±8.5)分,不同岗位医务人员情绪衰竭、消极怠慢和成就感得分差异均有统计学意义(均P=0.000);医务人员工作倦怠检出率为56.53%,其中轻、中、重度倦怠检出率分别为29.51%、21.44%、5.57%;不同岗位医务人员工作倦怠情况比较,护士中度工作倦怠检出率(25.05%)均高于医生(19.81%)和疾控人员(18.22%),差异均有统计学意义(均P=0.000);单因素logistic回归分析结果显示,城市、工作单位为三级医院/市级疾病预防控制中心、年龄30~39岁、工作年限10~19年、文化程度为大专及以上、职称为初级及以上的医务人员中、重度工作倦怠检出率较高(均P<0.01)。结论 辽宁省医务人员工作倦怠情绪普遍存在,城乡不同类型机构、不同岗位医务人员的工作倦怠率存在差异,急需采取综合措施进行防治。

     

    Abstract: Objective To assess levels and distribution of job burnout among medical staff in hospitals and health personnel in centers for disease control and prevention(CDC)in Liaoning province.Methods A total of 3 876 medical staff(1 807 doctors,1 405 nurses and 664 CDC staff)in Liaoning province were selected with multistage cluster random sampling to answer a self-designed questionnaire and Maslach Burnout Inventory-General Survey(MBI-GS)Chinese Version.Results The medical staff were scored with 11.1±6.8,5.5±5.3,and 23.0±8.5 for the dimensions of emotional exhaustion,cynicism,and personal accomplishment,respectively.The prevalence rate of low,moderate and high level of job burnout were 29.51%,21.44% and 5.57%,respectively.The prevalence rate of moderate burnout among the nurses(25.05%)was significantly hither than among the doctors(19.81%) and CDC staff(18.22%)(P=0.000).Results of binary logistic regression showed that the prevalence rate of moderate and high level burnout was significantly higher among the staff with the risk factors such as living in urban area,working in tertiary hospitals or city-level CDCs,at the age of 30-39 years,with work experience of 10-19 years,and having higher education or higher professional title(P<0.01 for all).Conclusion Job burnout exists among most medical and CDC staff in Liaoning.Significant differences in burnout exist among the medical staff living in different districts,working in medical fecilities at different levels,and with different job titles.Comprehensive measures should be taken urgently to prevent job burnout among medical staff.
