肺炎衣原体感染与肺癌血清学研究meta分析

Association of Cpn infection with lung cancer: a meta-analysis of serological studies

  • 摘要: 目的 综合评价肺炎衣原体(Chlamydia pneumoniae,Cpn)感染与肺癌之间的关系, 为肺癌病因研究和临床诊治提供科学依据。方法 检索Medline数据库、中国生物医学文献数据库、中国知识资源总库、维普资讯网、万方数据知识服务平台等多个中英文数据库, 收集Cpn感染与肺癌的病例对照研究文献, 同时利用所查文献及相关综述的参考文献、Google学术搜索进行滚动搜索补遗, 应用Stata 12.0软件进行meta分析。结果 共纳入12篇文献, 包括9篇英文文献和3篇中文文献, 累计2 557例病例人群和2 756名对照人群;meta分析结果显示, Cpn IgA抗体阳性和Cpn IgG抗体阳性与肺癌的合并OR值分别为2.025(95%CI=1.376~2.982)和1.774(95%CI=1.251~2.516), 吸烟者Cpn IgA和Cpn IgG阳性与肺癌的合并OR值分别为2.372(95%CI=1.463~3.847)和1.908(95%CI=1.112~3.275), Cpn IgA≥16、Cpn IgA≥64 和Cpn IgG≥16与肺癌的合并OR值分别为1.180(95%CI=1.029~1.353)、1.836(95%CI=1.098~3.068)和1.608(95%CI=1.022~2.531)。结论 Cpn 感染与肺癌发生有关, Cpn感染可增加吸烟者患肺癌的风险。

     

    Abstract: Objective To systematically evaluate the relationship of Chlamydia pneumoniae(Cpn) infection with lung cancer and to provide a scientific basis for etiology research and clinical diagnosis and treatment of lung cancer.Methods Medline,Sinomed,China National Knowledge Infrastructure,VIPdisc,WanFang Data and other databases were searched for the case-control studies on the association of Cpn infection with lung cancer.Meanwhile,Google Scholar was used to handsearch for addendum to the databases based on the references of the articles retrieved.Stata 12.0 software was applied in meta-analysis.Results Totally 12 eligible studies(9 English articles and 3 Chinese articles) were included,involving 2 557 lung cancer cases and 2 756 controls.The pooled odds ratios(ORs) for Cpn antibody IgA-positive and IgG-positive were 2.025(95% confidence interval 95%CI:1.376-2.982) and 1.774(95%CI:1.251-2.516).The pooled ORs for Cpn antibody IgA-positive and IgG-positive were 2.372(95%CI:1.463-3.847)and 1.908(95%CI:1.112-3.275)among smokers.Cpn infection was identified as a risk factor for lung cancer for the subjects with Cpn antibody IgA of ≥16 cutoff value(OR=1.180,95%CI:1.029-1.353),IgA of ≥64(OR=1.836;95%CI:1.098-3.068),and IgG of ≥16(OR=1.608,95%CI:1.022-2.531)Conclusion Cpn infection is associated with lung cancer,particularly among smokers.
