广东省2009-2014年霍乱弧菌耐药监测分析

Antibiotic resistance of Vibrio cholerae strains isolated in Guangdong province,2009-2014

  • 摘要: 目的 了解广东省2009-2014年霍乱弧菌耐药情况,为霍乱临床合理用药和防治提供科学依据。方法 采用WHO推荐的改良K-B纸片扩散法,对在广东省2009-2014年收集的253株霍乱弧菌进行12种抗菌药物的药敏试验。结果 广东省2009-2014年共收集分离霍乱弧菌O1群189株(小川型72株,稻叶型117株),O139群26株,非O1/O139群38株;药敏结果显示,253株霍乱弧菌对头孢噻肟(CTX)和头孢曲松(CRO)均100% 敏感,对诺氟沙星(NOR)、环丙沙星(CIP)、阿米卡星(AMK)的敏感率分别为98.8% 、98.4% 、98.0%;O1群小川型、O1群稻叶型、O139群、非O1/O139群霍乱弧菌比较,不同血清型霍乱弧菌对萘啶酸(NAL)、四环素(TCY)、多西环素(DOX)、复方新诺明(SXT)、氨苄西林(AMP)、阿莫西林(AMX)、氯霉素(CHL)等抗生素的耐药率差异均有统计学意义(均P<0.05);多重耐药分析结果显示,37.5% 的菌株同时对≥3种抗生素耐受,其中有12株O1群稻叶型菌株同时对≥6种抗生素耐受,有3株O1群小川型和1株非O1/O139群菌株同时对6种抗生素耐受,有2株O139群菌株同时对4种抗生素耐受。结论 CTX、CRO、NOR、CIP、AMK对霍乱弧菌的敏感率较高;非O1/O139群霍乱弧菌感染比例增高且部分菌株表现出多重耐药现象,应加强此类菌株的耐药监测。

     

    Abstract: Objective To study antibiotic resistance of Vibrio cholerae strains isolated in Guangdong province between 2009 and 2014 and to provide references for prevention and control of cholera.Methods Totally 253Vibrio cholera isolates were tested for the resistance to 12 selected antibiotics by the Kirby-Bauer method.Results Among the 253 isolates,189 strains belong to O1 serogroup(72 Ogawa serotype and 117 Inaba serotype);26 strains belong to O139 serogroup and 38 strains are non-O1 and non-O139 serogroups.Antibiotic susceptibility analysis showed that all the isolated strains were sensitive to cefotaxime and ceftriaxone and the sensitive rates of the isolated strains to norfloxacin,ciprofloxacin,and amikacin were 98.8%,98.4%,and 98.0%,respectively.The resistant rate of isolated Vibrio cholerae strains to nalidixic acid,tetracycline,doxycycline,trimethoprim-sulfamethoxazole,ampicillin,amoxicillin,and chloramphenicol differed significantly among the strains of three different serogroups(P<0.05).Multidrug resistance analysis showed that 37.5% of the strains isolated were resistant to three or more tested antibiotics,of which,12 strains of Inaba serotype were resistant to six or more antibiotics,while only 3 strains of Ogawa serotype and one strain of non-O1 and non-O139 serogroups were resistant to six drugs,2 strains of O139 serogroup were resistant to four drugs.Conclusion Vibrio cholerae stains isolated in Guangdong province are high sensitivity to cefotaxime,ceftriaxone,norfloxacin,ciprofloxacin,and amikacin.But the incidence of non-O1 and non-O139Vibrio cholerae infection increased and some multiple antibiotic resistant strains were detected,suggesting antibiotic resistance surveillance on Vibrio cholerae stains needs to be strengthened.
