丙泊酚和异氟烷对骨关节结核患者炎性作用评价

Effects of propofol and isoflurane on serum inflammatory factors in osteoarticular tuberculosis patients

  • 摘要: 目的 探讨丙泊酚和异氟烷对骨关节结核患者炎性作用的影响,为骨关节结核的临床治疗提供科学依据。方法 抽取2012年6月-2015年6月在内蒙古自治区扎兰屯市和辽宁省沈阳市某2所医院择期行病灶清除术的229例骨关节结核患者随机分成丙泊酚组和异氟烷组,丙泊酚组患者使用麻醉药物丙泊酚3~6μg/mL,异氟烷组患者使用麻醉药物1%~2%异氟烷;分别于麻醉前即刻、手术后即刻、手术后12 h、手术后24 h抽取2组患者静脉血5 mL,测定血清白细胞介素1(IL-1)、肿瘤坏死因子α(TNF-α)、细胞黏附分子1(ICAM-1)的含量。结果不同组别骨关节结核患者比较,丙泊酚组患者手术后12 h和手术后24 h的IL-1、TNF-α、ICAM-1含量分别为(12.42±4.36)和(19.82±7.08)ng/L、(79.62±30.54)和(89.53±34.61)μg/mL、(1.92±0.55)和(2.15±0.73)g/L,均低于异氟烷组患者的(15.78±5.71)和(24.36±7.63)ng/L、(86.72±31.45)和(97.20±36.58)μg/mL、(2.13±0.74)和(2.43±0.89)g/L(均P<0.01)。结论 麻醉药物丙泊酚较异氟烷可更好地减弱骨关节结核患者的炎性作用。

     

    Abstract: Objective To investigate effects of propofol and isoflurane on serum inflammatory factors in patients with osteoarticular tuberculosis.Methods Totally 229 inpatients undergoing osteoarticular tuberculosis focal cleaning in two hospitals in Shenyang city between June 2012 through June 2015 were divided randomly into a propofol and an isoflurane group and given 3-6μg/mL propofol and 1%-2% isoflurane for anesthetization of the operation,respectively.The patients' serum levels of interleukin-1(IL-1),tumor necrosis factor-α(TNF-α),and intercellular adhesion molecule-1(ICAM-1)were detected before the anesthesia,and at the beginning,12 and 24 hours after the operation.Results The serum levels of IL-1,TNF-α,and ICAM-1 were significantly higher in isoflurane group than those in propofol group at 12 and 24 hours after the operation(all P<0.05).The serum levels of IL-1,TNF-α,and ICAM-1 were significantly increased at the beginning,12 and 24 hours after the operation compared with those before the anesthesia in both groups(all P<0.01).Conclusion Compared to isoflurane,propofol is of greater inhibitive inflammation effect in patients with osteoarticular tuberculosis.
