合肥地区孕妇妊娠早期心理社会应激影响因素分析

Prevalence and influencing factors of psychosocial stress among pregnant women of first trimester in Hefei city

  • 摘要: 目的 描述孕妇妊娠早期心理社会应激流行特征及其影响因素,为妇女孕期心理卫生保健提供循证依据。方法 选择2011年5月-2013年4月在安徽省合肥市第一人民医院进行孕产期保健3826名孕27周以前的孕妇作为随访调查对象,分别采用《妊娠相关焦虑量表》、《爱丁堡产后抑郁量表》以及《妊娠期生活事件问卷》评价孕妇妊娠早期的妊娠相关焦虑、生活事件和抑郁发生情况。结果 3826名孕妇妊娠早期妊娠相关焦虑、抑郁和生活事件阳性检出率分别为30.4%、32.7%和9.3%;多因素logistic回归模型分析结果显示,完全意外怀孕(OR=2.355,95% CI=1.902~2.916)以及非初次怀孕(OR=1.176,95% CI=1.017~1.361)均为妊娠早期孕妇发生妊娠相关焦虑的危险因素,年龄25~29岁(OR=0.814,95% CI=0.675~0.981)、30~50岁(OR=0.706,95% CI=0.553~0.903)以及家庭月收入为5000~10000元(OR=0.833,95% CI=0.711~0.976)为妊娠早期孕妇发生妊娠相关焦虑的保护因素;完全意外怀孕(OR=1.487,95% CI=1.202~1.841)为妊娠早期孕妇发生抑郁症状的危险因素,家庭月收入为5000~10000元(OR=0.791,95% CI=0.679~0.923)以及文化程度为高中或中专(OR=0.761,95% CI=0.599~0.967)、大专(OR=0.729,95% CI=0.570~0.930)或本科及以上(OR=0.917,95% CI=0.544~0.951)为妊娠早期孕妇发生抑郁症状的保护因素;完全意外怀孕(OR=2.016,95% CI=1.464~2.775)为妊娠早期产生生活事件应激的危险因素,家庭月收入为5000~10000元(OR=0.757,95% CI=0.588~0.974)以及文化程度为本科及以上(OR=0.610,95% CI=0.392~0.949)为其保护因素。结论 孕妇妊娠相关焦虑、抑郁及生活事件应激较为普遍,妊娠意愿、妊娠年龄、孕妇文化程度以及是否初孕是孕期心理社会应激的主要影响因素。

     

    Abstract: Objective To explore psychosocial stress and its influencing factors in the pregnant women of first trimester pregnancy.Methods The participants of the study were 3826 pregnant women who took prenatal care in the First People's Hospital of Hefei City from May 2011 through April 2013.All participants were asked to complete a self-administered questionnaire survey to collect information on pregnancy related anxiety, depression and life events in the first trimester of pregnancy before 27 weeks of gestation.Results The detection rate of pregnancy-related anxiety, depression and life events were 30.4%, 32.7%, and 9.3%, respectively.The results multivariate logistic regression showed that unplanned pregnancy(odds ratio OR=2.355, 95% confidence interval 95% CI:1.902-2.916) and non-first pregnancy(OR=1.176, 95% CI:1.017-1.361) were the risk factors of pregnancy-related anxiety; the protective factors of pregnancy-related anxiety were aged 25-29 years(OR=0.814, 95% CI:0.675-0.981), aged 30-50 years(OR=0.706, 95% CI:0.553-0.903), and with a monthly familial income of 5000-10000 RMB Yuan(OR=0.833, 95% CI:0.711-0.976).The risk factor of depression was unplanned pregnancy(OR=1.487, 95% CI:1.202-1.841) and the protective factors were with a monthly familial income of 5000-10000 RMB Yuan(OR=0.791, 95% CI:0.679-0.923) and with an education of high school or technical secondary school(OR=0.761, 95%CI:0.599-0.967), with an education junior college(OR=0.729, 95% CI:0.570-0.930), and undergraduate or above(OR=0.917, 95% CI:0.544-0.951).Unplanned pregnancy(OR=2.016, 95% CI:1.464-2.775) was a risk factor of life event stress in the first trimester of pregnancy and the protective factors were with a monthly familial income of 5000-10000 RMB Yuan(OR=0.757, 95% CI:0.588-0.974) and with an education of bachelor degree and above(OR=0.610, 95% CI:0.392-0.949).Conclusion Pregnancy-related anxiety, depression and life event stress are common among the pregnant women in the first trimester of pregnancy and maternal age, education, pregnancy intention, and whether being pregnant for the first time are major influence factors of psychosocial stress.
