盘锦2014年室外空气污染对小学生呼吸系统健康影响

Health effect of outdoor air pollution on respiratory system among pupils in Panjin city, 2014: a cross-sectional survey

  • 摘要: 目的 了解辽宁省盘锦市2014年室外空气污染类型及对小学生呼吸系统健康影响情况。方法 根据盘锦市2010-2014年工业区、交通区、清洁区大气监测结果,2014年12月选取距空气监测点直线距离<1 km学校小学生共1 853人作为调查对象。采用国际统一标准问卷美国胸科协会(ATS)调查表。对小学生呼吸系统疾病和症状以及空气污染状况等方面进行调查。结果 工业区和交通区的PM10、二氧化硫、二氧化氮指标均高于清洁区(P<0.01)。持续咳嗽、持续咳痰、喘鸣比例与相应月份空气污染程度呈现正相关(P<0.05)。当沿街住房与交通干线的距离<5 m时,沿街居住的小学生患有呼吸道症状、持续咳嗽比例较高(OR=2.33,95%CI=1.18~4.61、OR=4.49,95%CI=2.18~9.27)。结论 交通污染、冬季燃煤污染是北方城市主要空气污染类型,空气污染可加重具有易感因素小学生呼吸系统疾病及症状的发生。

     

    Abstract: Objective To describe the type and variation of outdoor air pollution in Panjin city of Liaoning province and to explore the effect of the pollution on respiratory system in pupils.Methods Industrial,traffic and clean areas in Panjin city were identified based on air pollution monitoring data between 2010 and 2014.Totally 7 primary schools within one kilometer in straight-line distance from a monitoring sampling site for air pollution (2 in industrial,one in traffic and 2 in clean areas)were chosen as survey sites and 1 853 pupils recruited from the 7 schools were surveyed with the Respiratory Disease Questionnaire recommended by American Thoracic Society in December 2014.Results The concentrations of particulate matter less than 10 μm in aerodynamic diameter (PM10),sulfur dioxide (SO2),and nitrogen dioxide in ambient air of industrial and traffic areas were all significantly higher than those in clean areas (P<0.01 for all).Significant positive correlations were observed between levels of ambient air pollutants and reported incidences of persistent cough,persistent phlegm and wheeze among the pupils (all P<0.05).The pupils living in houses less than 5 meters away from a main traffic road reported higher incidences of respiratory symptoms and persistent cough,with the odds ratios (95% confidence intervals) of 2.33 (1.18-4.61) and 4.49 (2.18-9.27),respectively.Conclusion Traffic and coal-burning in winter are main air pollution sources in urban areas of northern China and ambient air pollution can aggravate incidence of respiratory diseases among pupils with susceptible factors.
