上海市老年人健康相关生命质量及其影响因素

Health-related quality of life and its influencing factors among elderly people in Shanghai city

      目的  了解上海市老年人健康相关生命质量及其影响因素,为提高老年人生存质量提供参考依据。
      方法  收集上海市2013年第五次国家卫生服务调查数据,对上海市17个区县11 103名老年人的健康相关生命质量及其影响因素进行分析。
      结果  上海市11 103名老年人中,欧洲五维健康量表(EQ-5D)健康描述系统行动能力、自我照顾能力、日常活动能力、疼痛或不舒服、焦虑或抑郁5个维度均没有任何困难者8 136人(73.28 %),有极度困难者16人(0.14 %)。上海市老年人EQ-5D量表视觉模拟标尺(EQ-VAS)得分为0~100分,25 %分位数为70分,中位数为80分,75%分位数为85分。多因素分位数回归分析结果显示,婚姻状况已婚和丧偶、每天吸烟、饮酒 ≥ 1次/周、体育锻炼 ≥ 1次/周和1年内进行健康体检的EQ-VAS得分Q5老年人得分较高,年龄 ≥ 70岁和患慢性病的EQ-VAS得分Q5老年人得分较低;婚姻状况已婚和丧偶、饮酒 ≥ 1次/周、体育锻炼 ≥ 1次/周和1年内进行健康体检的EQ-VAS得分Q25老年人得分较高,农业户口、年龄 ≥ 70岁、患慢性病和体育锻炼 < 1次/周的EQ-VAS得分Q25老年人得分较低;女性、饮酒 ≥ 1次/周和体育锻炼 ≥ 3次/周的EQ-VAS得分Q50老年人得分较高,年龄 ≥ 70岁和患慢性病的EQ-VAS得分Q50老年人得分较低;婚姻状况已婚、离异和丧偶的EQ-VAS得分Q75老年人得分较高,年龄 ≥ 70岁和患慢性病的EQ-VAS得分Q75老年人得分较低;农业户口和患慢性病的EQ-VAS得分Q95老年人得分较低。
      结论  上海市老年人健康相关生命质量较好,户籍类型、年龄、婚姻状况、是否患慢性病、吸烟情况、饮酒情况、体育锻炼和1年内是否进行健康体检是该地区老年人健康相关生命质量的主要影响因素。

     

    Abstract:
      Objective  To study the health-related quality of life (HRQoL) and its influencing factors among the elderly people in Shanghai municipality and to provide references for improving QoL of the elderly.
      Methods  The data on 11 103 residents aged ≥ 60 years living in 17 districts or counties of Shanghai municipality were extracted from the Fifth National Survey on Health Services sponsored by National Health and Family Planning Commission in 2013. The HRQoL of the elderly was evaluated with Euro-Quality of Life-5 Dimension (EQ-5D) and scored with EuroQol Group Visual Analogue Scale (EQ-VAS).
      Results  Of the 11 103 elderly people, 8 136 (73.28%) reported having no problems on all the five dimensions (mobility, self-care, usual activities, pain/discomfort, and anxiety/depression) of EQ-5D and 16 (0.14%) reported having extreme problems on all the dimensions. The EQ-VAS scores for all the elderly ranged from 0 to 100, with the score of 70 at 25% (Q25) quantile, the score of 80 at the median (50%, Q50), and the score of 85 at 75% (Q75) quantile, respectively. The quantile regression analyses demonstrated that the elderly with following characteristics were more likely to have a higher EQ-VAS score at specific quantile points: married or widowed, smoking everyday, drinking alcohol once a week or more, having physical exercise once a week or more, and having health checkup within a year at 5% (Q5) and Q25 quantile; being female, drinking alcohol once a week or more, and having physical exercise three times a week or more at Q50 quantile; and with the marital status of married, devoiced or widowed at Q75 quantile. Whereas, the quantile regression analyses revealed that the elderly with following characteristics were more likely to have a lower EQ-VAS score at specific quantile points: more than 70 years old and suffering from chronic diseases at Q5, Q50, and Q75 quantile; with agricultural registered permanent residence, more than 70 years old, suffering from chronic diseases, and having physical exercise less than one time at Q25 quantile; and with agricultural registered permanent residence and suffering from chronic diseases at 95% (Q95) quantile.
      Conclusion  The health-related quality of life is at a good level and mainly influenced by type of registered permanent residence, age, marital status, whether suffering from chronic disease condition, smoking, alcohol drinking, physical exercise, and whether having a health checkup within one year among the elderly people in Shanghai city.
