孕妇孕中期体力活动对婴儿智力发育影响前瞻性队列研究

Effects of maternal physical activity during mid-pregnancy on intelligence development of infants: a prospective cohort study

      目的  探讨孕妇孕中期体力活动对婴儿智力发育水平的影响,为母婴健康提供理论依据。
      方法  以安徽省铜陵市妇幼保健院为研究现场建立前瞻性队列研究,于2014年5月 — 2015年5月采用丹麦翻译的体力活动量表收集孕中期孕妇体力活动资料;纳入392名符合要求婴儿,其中男性172名,女婴220名,在2015年5月 — 2016年5月通过自编问卷收集婴儿喂养情况,并使用贝利婴幼儿发展量表对婴儿1岁时智力发育进行评估;运用逐步多元线性回归分析孕期体力活动对婴儿智力和体格发育水平的影响。
      结果  392名婴儿智力发育指数(MDI)平均为(126.39 ± 13.61)分,运动发育指数(PDI)平均为(113.61 ± 14.30)分;女婴的MDI得分高于男婴(P < 0.05),不同性别婴儿PDI得分差异无统计学意义(P > 0.05)。中度体力活动与婴儿MDI发育指标呈负相关(β = – 0.15,P = 0.013);轻度体力活动能量消耗与婴儿PDI发育指标呈正相关(β = 0.12,P = 0.048)。
      结论  孕妇孕中期适当的轻度体力活动与婴儿PDI指标呈正相关,中度体力活动与婴儿1岁时智力发育MDI指标呈负相关,提示妊娠期妇女体力活动应适当。

     

    Abstract:
      Objective  To explore the influence of maternal physical activity during mid-pregnancy on infants′ intelligence development and to provide theoretical evidences for promoting maternal and infant health.
      Methods  Using prospective cohort study design, we recruited 392 singleton pregnant women having routine antenatal examination at Tongling Maternal and Child Health Hospital and followed them up to delivery from May 2014 to May 2015; we collected relevant data with a questionnaire interview and information on physical activity in middle stage of pregnancy using a scale developed by Danish researchers. We also performed a one-year follow up survey among 392 infants (172 males and 220 females) of the women from May 2015 to May 2016. We collected the infants′ information on feeding using a self-designed questionnaire and on mental development at age of 12-month with Chinese Revision of Bayley Scale of Infant Development (BSID-CR). The effects of maternal physical activity on the infants′ intelligence and physical development were analyzed with stepwise multivariate linear regression.
      Results  The average mental developmental index (MDI) and psycho-motorial developmental index (PDI) were 126.39 ± 13.61 and 113.61 ± 14.30 for all the infants; the MDI of the female infants was significantly higher than that of male infants (P < 0.05); while the PDI of the infants did not differ significantly by gender (P > 0.05). Moderate maternal physical activity during mid-pregnancy of the pregnant women was reversely correlated with the infants MDI (β = – 0.15, P = 0.013); whereas slight physical activity is positively correlated with infants′ PDI (β = 0.12, P = 0.048).
      Conclusion  For the women during mid-pregnancy, slight physical activity is positively correlated with their infants′ psycho-motorial developmental index but moderate physical activity is reversely correlated with the infants′ mental developmental index at age of 12-month. The results suggest that pregnant women should maintain appropriate physical activity.
