河北省死因监测点居民2014 — 2019年心脏病死亡趋势分析

Heart disease mortality among residents in Hebei province, 2014 – 2019: an analysis on death registry data

      目的  了解河北省死因监测点居民2014 — 2019年心脏病死亡趋势,为采取相应的干预措施提供参考依据。
      方法  收集河北省人口死亡信息登记管理系统平台中监测点居民2014年1月1日 — 2019年12月31日上报的心脏病死亡数据,分析河北省死因监测点不同性别和年龄居民2014 — 2019年心脏病的死亡趋势。
      结果  河北省死因监测点居民2014、2015、2016、2017、2018和2019年心脏病死亡率分别为186.85/10万、180.18/10万、184.17/10万、192.98/10万、214.01/10万和207.06/10万,标化死亡率分别为191.26/10万、185.50/10万、172.31/10万、173.83/10万、178.89/10万和170.56/10万,2014 — 2019年心脏病标化死亡率呈下降趋势(χ2趋势 = 171.553,P < 0.001),男性和女性心脏病标化死亡率亦均呈下降趋势(χ2趋势 = 51.327和119.630,均P < 0.01);2014 — 2019年河北省死因监测点35岁~和85岁~年龄组居民心脏病死亡率均呈上升趋势(均P < 0.05),20岁~、40岁~、60岁~、65岁~、70岁~、75岁~和80岁~年龄组居民心脏病死亡率均呈下降趋势(均P < 0.01);心脏病死亡比例从高到低的类型依次为缺血性心脏病(69.17 %)、高血压性心脏病(19.79 %)、其他类型心脏病(8.95 %)、风湿性心脏病(1.66 %)和炎性心脏病(0.44 %)。
      结论  河北省死因监测点居民2014 — 2019年心脏病总体死亡率呈下降趋势,缺血性心脏病是最主要的死亡类型。

     

    Abstract:
      Objective  To examine the trend in heart disease mortality among the residents in mortality surveillance points in Hebei province from 2014 to 2019, and to provide references for developing relevant intervention measures.
      Methods  From Hebei Province Death Registration Management System, we extracted all records of heart disease death from January 1, 2014 to December 31, 2019 among residents living in regions covered by the Mortality Surveillance System. The collected data were analyzed using descriptive statistics methods.
      Results  Among the population in 30 mortality surveillance points across the province, totally 173 521 heart disease deaths were registered during the 6-year period. The annual heart disease mortality rate (1/100 000) for the years of 2014 – 2019 were 186.85, 180.18, 184.17, 192.98, 214.01, and 207.06 and the standardized mortality rate were 191.26, 185.50, 172.31, 173.83, 178.89, and 170.56, respectively. For the 6-year period, there was a significant downward trend in overall and gender-specific standardized heart disease mortality rate (for total population: χ2trend = 171.553, P < 0.001; for males and females: χ2trend = 51.327 and 119.630, both P < 0.01); the age-specific heart disease mortality rate presented an upward trend for the residents aged 35 – 39, and ≥ 85 years (all P < 0.05) but a downward trend for the residents aged 20 – 40, 40 – 44, 60 – 64, 65 – 69, 70 – 74, 75 – 79 and 80 – 84 years (all P < 0.01). For all the heart disease mortalities, the constituent ratio was 69.17% for ischemic heart disease, 19.79% for hypertensive heart disease, 1.66% rheumatic heart disease, 8.95% for other heart diseases, and 0.44% for inflammatory heart disease, respectively.
      Conclusion  From 2014 to 2019, the heart disease mortality showed a downward trend generally among the residents in mortality surveillance points in Hebei province and ischemic heart disease was a main contributor to the mortalities.
