内蒙古自治区五盟市农业环境因素与肺癌患病风险空间关联分析

Association between agricultural environment and lung cancer risk in five areas of Inner Mongolia: a spatial correlation analysis

      目的  了解内蒙古自治区巴彦淖尔市、鄂尔多斯市、包头市、呼和浩特市、乌兰察布市5个盟市农业环境因素与肺癌患病风险的空间关联性,为肺癌的防控工作提供参考依据。
      方法  收集内蒙古自治区5个盟市2010年10月 — 2011年12月的人群健康档案数据及同期人口学数据和宏观经济社会数据,采用流行病学和空间统计分析技术相结合的方法分析宏观农业环境因素与肺癌患病风险之间的关联及其区域差异。
      结果  内蒙古自治区巴彦淖尔市、鄂尔多斯市、包头市、呼和浩特市、乌兰察布市5个盟市43个区县(旗)居民肺癌总患病率为9.57/10万,其中男性和女性肺癌患病率分别为12.60/10万和6.22/10万;肺癌与农业因子的地理加权回归分析结果显示,在5个盟市区域上,从东南向西北肺癌患病风险与农业因子间的正相关关系呈逐渐增强趋势,特别是巴彦淖尔市的乌拉特后旗肺癌患病风险与农药使用量、化肥施用量、农用塑料薄膜使用量的综合农业因子之间存在空间关联。
      结论  内蒙古自治区5个盟市农业环境因素与肺癌患病风险间存在空间关联及区域差异,随着农药使用量、化肥施用量和农用塑料薄膜使用量的增加,肺癌患病风险随之升高。

     

    Abstract:
      Objective  To explore spatial correlation between agricultural environmental factors and lung cancer risk in five prefectures of Inner Mongolia Autonomous Region (Inner Mongolia), and to provide a reference for the prevention and control of lung cancer.
      Methods  Health records registered between October 2010 and December 2011 for the residents in 43 districts/counties/banners of five prefectures (Bayannur, Ordos, Baotou, Hohhot, and Ulan Chabu) of Inner Mongolia were collected and the data on demographics and macro economy for the regions during the period were extracted from the Sixth National Census and Social Economic Survey Yearbook of Inner Mongolia – 2012 simultaneously. Epidemiological analysis combined with space statistics was performed to assess the correlation between lung cancer risk and agricultural environmental factors and regional disparities in the correlation.
      Results  During the period among 11 271 137 residents in the five prefectures, totally 1 079 lung cancer cases were registered, with the prevalence rates of 9.57/100 000 for the all residents and 12.60/100 000 and 6.22/100 000 for the male and female residents, respectively. The results of geographically weighted regression (GWR) analysis showed that the positive correlation between lung cancer risk and agricultural factors gradually strengthened for the five prefectures of different geographical locations from southeast to northwest. Close spatial associations of pesticide, chemical fertilizer, and agricultural plastic film use with lung cancer risk were observed in a banner of Bayannur prefecture.
      Conclusion  In five prefectures of Inner Mongolia, agricultural environmental factors are associated with lung cancer risk and use of pesticides, chemical fertilizers and agricultural plastic film could increase lung cancer risk.
