不同营养状况儿童心脑血管疾病危险因素水平分析

Cardiovascular and Cerebrovascular Disease Risk Factors in Children of Different Nutritional Conditions

  • 摘要: 目的探讨在不同营养状况儿童中心脑血管疾病风险因素水平变化.方法在北京城区4所小学中随机选取317名8~11岁健康儿童,进行身体测量及血脂、胰岛素检测,记录家庭成员心脑血管疾病史.结果营养不良与超重肥胖的检出率分别为13.9%和24.0%,上述两组与正常组相比,一、二级亲属心脑血管疾病患病率明显增加(P<0.01),超重肥胖组与营养不良和正常组相比血压及胰岛素水平显着升高(P<0.01),血脂谱中TG升高,HDL/LDL、apoA/apooB下降(P<0.01),超重肥胖组与正常组相比TC和lp(a)升高(P<0.05),逐步回归分析得出BMI与收缩压、TG、胰岛素呈正相关关系,与HDL呈负相关关系.结论超重、肥胖和营养不良儿童心脏血管疾病危险因素水平均高于正常儿童.

     

    Abstract: ObjectiveTo explore the change of car diovascular and cerebrovascular disease factors in children of different nutritional conditions.Methods317 health children aged 7~11 years were selected randotmly from 4 primary schools in Beijing.Anthropometry was carr ied out for all of them.Fasting insulin and lipids were measured and family history of the cardiovascular and cerebrovascular disease was recorded for them.Results13.9% of the children was malnutrition(defined as weight for height <90% of the ideal weight).24.0% of the children was overweight(defined as weight for height>1.0% of the ideal weight).The prevalence of the car diovascular and ceredrovescular disease was significantly higher in family members of malnur ition and overweight children compared with ideal weight children(59.5% and 45.7% Vs 33.9% P<0.01).The over weight children showed higher age and sex-adjusted mean vaiue of blood pressure,fasLing insulin,triglyceride,and lower value of HDL/LDL,apoA/apoB than nonoverw eight(P<0.01)and higher value of cholesterol and lipoprotein(a)than ideal weight children(P<0.05).Multiple linear regression analysis revealed that BMI was positive correlation with systo lic pressure,fasting insulin and triglyceride,and negative correlation with HDL.ConclusionCardiovascular and cerebrovascular disease factors were higher in malnutr ition and overweight children than ideal weight children.
