多氯联苯与乳腺癌关系研究的Meta分析

Meta analysis on relationship between PCBs and human breast cancer

      目的   综合分析人体内多氯联苯(PCBs)暴露与女性乳腺癌的关系。
      方法   查询筛选合格的PCBs与人类乳腺癌关系的病例对照研究全文13篇(1999~2003年)。用Meta分析方法进行计算, 对具有同质性的资料采用M-H法估计总效应和95%CI, 对异质性的资料采用D-L法估计总效应和95%CI。按生物样品种类、病例的绝经状态和易感基因分型3个亚组进行分析。
      结果   13项研究结果一致性检验统计量Q=23.33 > χ(0.05, 12)2, 研究间具有异质性(P < 0.05), 用D-L计算出OR综合=1.05, 95%CI=0.99~1.20。非绝经组OR综合=1.40, 95%CI=1.01~1.96;突变基因组的OR综合=1.89, 95%CI=1.21~2.97。
      结论   在非绝经妇女中PCBs高暴露可能是乳腺癌发生的危险因素。携带雌激素代谢酶CYP1A1突变基因型的人群可能是PCBs高暴露致乳腺癌的易感人群。

     

    Abstract:
      Objective   To analyze relationship between polychlor inated biphenyl(PCBs)exposure and breast cancer.
      Methods   13 research reports of case-control studies were collected.Meta analysis method was used to analyze total ORs.A fixed effect model(FEM)or a random effect model(REM)was chosen to calculate total ORs depending on the results of the test of homogeneity(Qtest).Three subgroups, including source of biomaterials, menopausal status, and polymorphism of CYP1A1 gene, were analyzed to explore heterogeneity.
      Results   The Q value for the test of homogeneiy of the 13 reports was 23.33(P < 0.05).The value of ORDL calculated by REM was 1.05(95%CI, 0.99~1.20).The results of subgroup analysis showed five subgroups were homogeneity(P > 0.05).Values of total ORs of the subgroups of premenop ausal, CYP1A1 gene mutation were 1.40(95%CI, 1.01~1.96)and 1 89(95% CI, 1.21~2.97)respectively.
      Conclusion   Premenopausal women with PCBs high exposure may be a high-risk population for attacking breast cancer.Subg roup with variant genotypes of CYP1A1 may be the susceptible population for PCBs exposure.
