摘要: 目的 了解辽宁省沈阳市不同级别综合医院内科门诊病人各种焦虑障碍的患病率,并进行比较。方法 采用分层随机整群抽样方法抽取沈阳市不同级别的23家综合医院进行调查,共调查年龄≥15岁的内科门诊病人5312例。调查员在病人内科就诊前使用扩展的一般健康问卷(GHQ-12)作为筛查工具,将就诊患者分为精神疾病高、中、低危险组,然后由精神科医生使用美国《精神障碍诊断与统计手册》第4版轴I障碍定式临床检查(SCID-I/P)对高、中、低危险组100%,40%,10%的调查对象进行诊断。结果 沈阳市三级综合医院内科门诊患者各种焦虑障碍的总现患率为5.93%(95%CI=4.98%~7.05%),高于二级综合医院的3.96%(95%CI=3.12%~5.02%),RR值为0.67(95%CI=0.50~0.90)。结论 沈阳市三级综合医院内科门诊各种焦虑障碍总患病率显著高于二级医院,应该更加关注三级综合医院内科门诊焦虑障碍患者的诊治问题。Abstract: Objective To describe the prevalence of anxiety disorders of outpatients in internal medicine departments in general hospitals of different level in Shenyang.Methods Stratified multi-stage cluster randomization was used to identify 23 general hospitals for the investigation.A total of 5 312 outpatients above 15 years old in internal medicine departments were investigated.Expanded version of General Health Questionnaire(GHQ-12)was used as a screen instrument to identify subg roups of the sample at high,moderate and low risk,then the psychiatrists deter mined heir diagnosis by using structured clinical interview for DSM-IVAx is I Disorders-Patient Edition(SCID-I/P)for 100% of high-risk and 40% of moder ate-risk and 10% of low-risk subjects.Results The overall current prevalence of anxiety disorders in outpatients of level 3 hospitals(5.93%,95%CI:4.98%~7.05%)was higher than that of level 2(3.96%,95%CI:3.12%~5.02%),RR was 0.67(95%CI:0.50~0.90).Conclusion The prevalence of anxiety disorders in outpatients in internal medicine departments of general hospitals in level 3 was higher than that of level 2.
