青岛市原发性肝癌影响因素病例对照研究

Related factors of hepatocellular carcinoma among Qingdao citizens: a case-control study

  • 摘要: 目的 探讨山东省青岛市原发性肝癌的主要影响因素,为青岛市原发性肝癌的病因预防和人群防治提供依据.方法 对青岛市194例原发性肝癌和同时期、同地区的健康人群210人进行成组病例-对照研究,通过单因素和多因素非条件Logistic回归分析筛选原发性肝癌的主要影响因素.结果 单因素分析结果显示,肝癌危险因素有HBsAg阳性、个人肝炎史、肝炎家族史、食用肥肉和烟熏食品;保护因素有食用鲜鱼、大蒜和洋葱,饮茶、性格外倾性和心情愉快(P<0.05);多因素分析结果表明,肝癌的主要危险因素有HBsAg阳性(OR=23.370,95%CI=12.625~43.258)、个人肝炎史(OR=5.188,95%CI=1.888~14.258)、食用烟熏食品(OR=3.082,95%CI=1.469~6.467)和肥肉(OR=2.685,95%CI=1.458~4.943);保护因素有饮茶(OR=0.533,95%CI=0.292~0.973)、鲜鱼(OR=0.448,95%CI=0.224~0.895)、心情愉快(OR=0.305,95%CI=0.131~0.713).结论 HBsAg阳性、个人肝炎史、嗜好烟熏食品和肥肉可能是青岛市原发性肝癌的主要危险因素;饮茶、鲜鱼、心情愉快可能是原发性肝癌的保护因素.

     

    Abstract: Objective To study related factors of hepatocellular carcinoma in population of Qingdao city and to provide scientific basis for control and prevention of hepatocellular carcinoma.Methods A case-control study involving 194 hepatocellular carcinoma cases and 210 age-,community-matched health controls was carried out.The main related factors were analyzed with logistic regression model and evaluated by odds ratio(OR) and 95% confidence interval(95% CI).Results Single factor analyses showed that hepatits B surface antigen(HBsAg) positive,personal history of hepatitis,family history of liver cancer,and intake of fat and smoked food were the main risk factors of hepatocellular carcinoma.But intakes of fresh fish,garlic and onion,tea,outgoing personality and happiness were the protective factors.Multiple factor analyses showed that HBsAg positive(OR=23.370,95% CI:12.625-43.258),personal history of hepatitis(OR=5.188,95% CI:1.888-14.258),intake of smoked food(OR=3.082,95% CI:1.469-6.467),and intake of fat(OR=2.685,95% CI:1.458-4.943) were the main risk factors of hepatocellular carcinoma.But drinking tea(OR=0.533,95% CI:0.292-0.973),taking fresh fish(OR=0.448,95% CI:0.224-0.895),and happiness(OR=0.305,95% CI:0.131-0.713) were the protective factors.Conclusion HBsAg positive,personal history of hepatitis,intakes of smoked food and fat may be the main risk factors of hepatocellular carcinoma,whereas,consumption of tea and fresh fish and happiness may be the proctective factors.
