基层人员糖尿病综合防治能力培训效果评价

Effects of training on comprehensive control and treatment of diabetes among community health workers

  • 摘要: 目的 了解基层公共卫生工作者糖尿病综合防治能力及培训效果,为进一步提升基层糖尿病管理服务水平提供参考依据。方法 在北京、上海、重庆、广东、浙江五省/直辖市的区县疾病预防控制中心、社区卫生服务中心(站)和综合医院范围内招募500名基层公共卫生工作者,进行为期2天的糖尿病综合防治能力培训。在培训前、后均进行问卷调查了解培训效果。结果 基层公共卫生工作人员培训知识考评总分由培训前的(74.29±15.63)分提高到(85.84±11.50)分,差异有统计学意义(P<0.05);疾控系统人员培训后培训知识考评总分为(88.89±10.62),高于综合医院工作人员的(80.95±12.32)分,差异有统计学意义(P<0.05);基层公共卫生工作人员糖尿病流行的原因知晓率培训前后分别为97.7%和98.5%、2型糖尿病的危险因素分别为98.1%和97.3%;培训后控烟政策知晓率由12.6%上升至20.6%,胰岛素注射技巧知晓率由54.1%上升至72.8%,差异均有统计学意义(P<0.05);95.4%的调查对象对培训的总体评价为非常满意或满意。结论 通过短期培训基层公共卫生工作者糖尿病综合防治知识和技能明显提高,疾控系统和社区卫生服务机构人员在社区糖尿病患者管理中的优势较突出。

     

    Abstract: Objective To evaluate the effects of training on comprehensive control and treatment of diabetes and ability of diabetes management and service among community health workers (CHWs)and to provide reference for improving diabetes patient management at grassroots level.Methods A two-day training course focusing on diabetes comprehensive control and prevention was delivered to 500 CHWs randomly selected from centers for disease control and prevention(CDCs),community health centers,and general hospitals at district or county levels in Beijing,Shanghai,and Chongqing city and Guangdong and Zhejiang province.The efficacy of the training was evaluated pre/post the training course through a questionnaire survey.Results There was a statistically significant increase in the score of diabetes related knowledge (74.29±15.63 vs 85.84±11.50,P<0.05).There were increases in the ratio of general knowledge of epidemic (95.5% vs 97.7%)and risk factors of diabetes (98.1% vs 97.3%)after the training.Awareness rate of tobacco control policies increased significantly from 12.6% to 20.6% (P<0.05).There were 72.8% of the CHWs knowing insulin injection skills,significantly higher than 54.1% before the training (P<0.05)and 95.4% of the CHWs expressed their satisfaction with the training course.Conclusion CHWs' diabetes management capability was improved after the training.CHWs from CDCs and community health centers have an advantage over those from general hospitals to deliver diabetes management at community level.
