HBsAg阳性孕妇细胞转运意义的研究

Study on HBsAg positive pregnant women cell transportion

  • 摘要:
      目的   了解HBsAg阳性孕妇细胞是否可进入胎儿血循环, 探索母儿细胞转运与乙型肝炎病毒(HBV)宫内感染关系。
      方法   用等位基因特异性PCR(allele specfic PCR, ASPCR)和半巢式PCR(hemin PCR)技术扩增新生儿外周血中母亲-S-DNA, 判定母儿细胞转运。非条件Logistic回归模型分析母儿细胞转运与HBV宫内感染关系。
      结果   以谷胱甘肽转移酶(GST)M1、血管紧张素转换酶(ACE)基因作为母亲的特异性等位基因, 检测42对信息病例对中26例(61.90%, 26/42)新生儿发生了母儿细胞转运。母儿细胞转运、孕妇外周血单个核细胞(PBMC)HBVDNA阳性是HBV宫内感染的危险因素, OR值分别为8.40(1.42~49.74), 8.78(1.23~62.69)。
      结论   母儿细胞转运可能是HBV宫内感染的重要原因, PBMC可能介导HBV穿过胎盘屏障感染胎儿。

     

    Abstract:
      Objective   To study the route that the HBsAg positive pregnant women's cell entered fetal circulation, and to probe into the association between the cell transportion from mother to baby and HBV intrauterine infection.
      Methods   The cell transportion from mother to baby were deter mined by allele-specific PCR(AS-PCR)and heminested PCR(hemi-nPCR).The association between the cell transportion from mother to baby and HBV intratuerine infection was analyzed by noncondition Logistic regression model.
      Results   GSTM 1, ACE as maternal specific alleles: 42 mothe-rbaby pairs were recruited from hemocyte specimens as informative cases, 26(61.09%)newhorns.blood cell out of 42 informative cases had maternal cell DNA.The cell transportion from mother to baby and PBMC HBV infection in pregnant women were risk factors of HBV intrauterine infection, OR were respectively 8.40(1.42-49.74)、(OR=8.78, 1.23-62.69).
      Conclusion   s The cell transportation from HBsAg positive mothers to baby had more contribution to HBV intrauterine infection; HBV infection fetus was probably mediated by PBMC to pass through the placentae barrier and infect fetus.
