北京市接种门诊女性工作者HPV相关知识、态度和疫苗接种情况调查

Investigation Report and Analysis Knowledge, attitudes and vaccination status regarding HPV vaccines among female healthcare workers at vaccination clinics in Beijing city: a survey report

    目的 了解北京市预防接种门诊中参与人乳头瘤病毒(HPV)疫苗接种服务的女性工作人员对HPV疫苗认知水平、态度及接种行为,为提升北京市HPV疫苗接种率提供参考。
    方法 2023年6月通过工作群向北京市所有承担HPV疫苗接种服务门诊中参与HPV疫苗接种服务的女性工作人员推送自行设计的问卷星问卷,对其HPV疫苗相关知识、态度和接种情况进行横断面调查,使用χ2检验和logistic回归方法对HPV疫苗接种行为的相关影响因素进行分析。
    结果 本次调查共回收有效问卷1 902份,占北京市预防接种门诊工作人员总数的17.24%(1 902/11 030),HPV疫苗接种比例为70.77%。调查对象来自全市实际承担HPV疫苗接种服务的预防接种门诊,门诊覆盖率64.09%(496/774)。HPV认知综合评分中位数为9分(IQR = 8~10)。1 520名(79.92%)调查对象对HPV疫苗接种持积极态度。二元logistic回归模型结果显示,接种HPV疫苗态度综合评分(OR = 2.430,95%CI = 1.873~3.153)、认为疫苗价格较高(OR = 0.616,95%CI = 0.427~0.888)、年龄(与 ≥ 50岁研究对象相比,< 30岁、30~39岁、40~49岁研究对象的OR值分别为33.110、17.047和13.141)以及工作年限(相对于 < 5年的人员,工作5~9年、10~14年,≥ 15年人员OR值分别为1.639、2.063和1.498)是影响HPV疫苗接种的主要因素。
    结论 北京市预防接种门诊中参与HPV疫苗接种服务的女性工作人员对HPV一般知识掌握较好,态度积极,接种率相对较高,为北京市做好HPV预防接种工作提供有力保障。通过持续开展全市HPV疫苗接种服务人员的专业培训和宣传工作,降低疫苗价格等措施,可有效提高HPV疫苗接种率。

     

    Abstract:
    Objective To understand the knowledge level, attitudes, and vaccination behaviors regarding Human papillomavirus (HPV) vaccines among female healthcare workers providing HPV vaccination services in Beijing′s immunization clinics, and to provide references for improving HPV vaccination rates in Beijing.
    Methods In June 2023, a self-designed online questionnaire was distributed through work groups to female healthcare workers involved in HPV vaccination services at all Beijing immunization clinics providing HPV vaccines. A cross-sectional survey was conducted to assess their HPV vaccine-related knowledge, attitudes, and vaccination status. Chi-square tests and logistic regression were used to analyze factors associated with HPV vaccination behavior.
    Results A total of 1 902 valid questionnaires were collected, representing 17.24% (1 902/11 030) of immunization clinic staff in Beijing, with an HPV vaccination rate of 70.77%. Respondents came from immunization clinics actually providing HPV vaccination services, with a clinic coverage rate of 64.09% (496/774). The median HPV knowledge composite score was 9 (IQR = 8 – 10). 1 520 respondents (79.92%) held positive attitudes toward HPV vaccination. Binary logistic regression analysis showed that the main factors affecting HPV vaccination were: attitude composite score (OR = 2.430; 95%CI = 1.873 – 3.153), perception of high vaccine price (OR = 0.616, 95%CI: 0.427 – 0.888), age (compared to ≥ 50 years, OR values for < 30, 30 – 39, and 40 – 49 years were 33.110, 17.047, and 13.141, respectively), and years of work experience (compared to < 5 years, OR values for 5 – 9, 10 – 14, and ≥ 15 years were 1.639, 2.063, and 1.498, respectively).
    Conclusions Female healthcare workers providing HPV vaccination services in Beijing′s immunization clinics demonstrated good general knowledge of HPV, positive attitudes, and relatively high vaccination rates, providing strong support for HPV vaccination work in Beijing. Vaccination rates can be effectively improved through continued professional training and education of vaccination service personnel citywide and reducing vaccine prices.
