广州市白云区钟落潭镇社区老年人衰弱与天然齿数关系

Relationship between frailty and number of natural teeth among community-dwelling elderly in a district of Guangzhou city, 2021: a cross-sectional survey

    目的 了解广东省广州市白云区钟落潭镇社区老年人衰弱情况及其与天然齿数的关系,为社区老年人的衰弱预防及健康管理提供数据资料和参考依据。
    方法 于2021年 9 — 10月采用随机整群抽样方法在广州市白云区钟落潭镇抽取12个社区共1342名 ≥ 65岁社区老年人进行问卷调查和口腔检查,并应用多因素非条件logistic回归模型分析该地区社区老年人衰弱与天然齿数之间的关系。
    结果 广州市白云区钟落潭镇最终纳入分析的1153名 ≥ 65岁社区老年人中,224人处于衰弱状态,衰弱率为19.4%;有0颗天然齿者207人(18.0%),1~10颗天然齿者184人(16.0%),11~20颗天然齿者239人(20.7%), > 20颗天然齿者523人(45.3%);在调整了性别、年龄、是否受过教育、月平均收入、是否独居、吸烟情况、饮酒情况、住房近2年是否装修和是否使用生物质燃料等混杂因素后,多因素非条件logistic回归分析结果显示,广州市白云区钟落潭镇有1~10颗、11~20颗和 > 20颗天然齿老年人衰弱风险分别为有0颗天然齿老年人的0.57倍(OR = 0.57,95%CI = 0.35~0.92)、0.64倍(OR = 0.64,95%CI = 0.41~0.99)和0.37倍(OR = 0.37,95%CI = 0.25~0.54)。
    结论 广州市白云区钟落潭镇社区老年人衰弱与其天然齿数有关,天然齿数颗数较多老年人的衰弱率较低。

     

    Abstract:
    Objective To understand the prevalence of and association of the number of natural teeth with frailty among the community-dwelling older people in Guangzhou city, and to provide evidence for frailty prevention of and health management in the population.
    Methods A face-to-face questionnaire survey and oral examination were conducted among 1 342 elderly residents (aged 65 years and older) recruited through random cluster sampling in a community in a district of Guangzhou city, Guangdong province, from September to October 2021. Multivariate unconditional logistic regression analysis was used to analyze the relationship between frailty and the number of natural teeth in older people.
    Results Of the 1 153 participants finally included in the analysis, 224 (19.4%) were classified as frail, and the numbers (percentages) of participants with zero, 1 – 10, 11 – 20, and more than 20 natural teeth were 207 (18.0%), 184 (16.0%), 239 (20.7%), and 523 (45.3%), respectively. After adjustment for sex, age, education, average monthly personal income, living alone, smoking, alcohol consumption, home renovation in the last 2 years, and use of biomass fuel, multivariate logistic regression results showed that participants with a higher number of natural teeth had a reduced risk of frailty compared to those with no natural teeth, with odds ratios (95% confidence interval) of 0. 57 (0.35 – 0.92), 0.64 (0.25 – 0.99), and 0.37 (0.25 – 0.54) for those with 1 – 10, 11 – 20, and >20 natural teeth, respectively.
    Conclusion The number of natural teeth is inversely correlated with the prevalence of frailty among community-dwelling older people in Guangzhou city.
