中国县级疾控机构卫生人力资源配置趋势及公平性:基于2019—2023年统计数据分析

Trends and equity of health human resource allocation in the county-level centers for disease control and prevention in China from 2019 to 2023

    目的 分析中国县级疾病预防控制机构人力资源配置现状及其公平性,为促进县级疾控事业高质量发展提供参考。
    方法 基于全国疾控综合管理系统数据及2019—2023年统计年鉴数据,运用基尼系数、泰尔指数以及卫生资源密度指数,从人口、地理、经济等维度,对全国县级疾控机构人力资源配置现状和公平性进行分析。
    结果 2023年我国县级疾控人员总数达162 836人,每万人口疾控人员数1.16人,均呈逐年增长趋势。各地区基尼系数按人口分布均<0.2,按经济水平分布均<0.3,按地理面积分布东、中部地区均<0.4,西部地区接近0.6。各地区泰尔指数在地理维度上数值约为0.36,人口和经济维度上约0.02及0.07,人口和地理维度区域内贡献率高于区域间,经济维度相反。全国及各地区卫生资源密度指数呈现逐年递增趋势。
    结论 我国县级疾控机构人员数量持续增长,按人口与经济维度配置较为均衡,地理维度配置公平性较弱,应推动区域内人力资源协调整合,缩减区域内部配置差异。

     

    Abstract:
    Objective To analyze the current status and equity of human resource allocation in centers for disease control and prevention (CDCs) at the county level in China, thereby providing reference for promoting the high-quality development of county-level CDCs.
    Methods The data were collected from the National Disease Prevention and Control Integrated Management System and the 2019–2023 Statistical Yearbooks. The Gini coefficient, Theil index, and health resource density index were adopted to analyze the current status and equity of human resource allocation in China′s county-level CDCs in the dimensions of population, geography, and economy.
    Results In 2023, the staff size and ratio in China′s county-level CDCs reached 162 836 and 1.16 per 10 000, respectively, both demonstrating consistent year-on-year growth. The Gini coefficient of all regions was less than 0.2 according to population distribution, less than 0.3 according to economic level distribution, less than 0.4 in eastern and central China and close to 0.6 in western China according to geographical area distribution. The Theil index across regions was approximately 0.36 in the geographical dimension, and 0.02 and 0.07 in the population and economic dimensions, respectively. For population and geographical dimensions, the intra-regional contribution exceeded that between regions, while the opposite pattern held for the economic dimension. The national and regional health resource density index showed an increasing trend year by year.
    Conclusions The staff size in China′s county-level CDCs has shown sustained growth. Their distribution remains relatively equitable across population and economic dimensions, but exhibits weaker equity in the geographical dimension. Efforts should focus on promoting intra-regional coordination and integration of human resources to reduce disparities within regions.
