浙江省公共卫生安全均衡性分析:基于熵权TOPSIS法

Equilibrium of public health security in Zhejiang province based on entropy weight TOPSIS

    目的  了解浙江省县(市、区)层面不同特征地区公共卫生安全水平均衡性现状,为全省精准施策提供依据。
    方法  根据公共卫生安全评价指标体系,收集浙江省7个县(市、区)2023年公共卫生安全相关数据,采用熵权逼近理想解排序(TOPSIS)计算每个县(市、区)的指标值。再根据不同经济社会发展水平、不同地形、不同县级行政类别分组计算政策保障、资源支撑、健康环境、监测预警、应急处置和脆弱人群6个维度的综合贴近度。采用变异系数计算公共卫生安全发展水平的均衡程度。
    结果  浙江省中等发达地区的公共卫生安全水平综合贴近度最高(0.644 0),且远高于发达地区(0.399 3)和欠发达地区(0.344 6),其中健康环境维度在3个不同经济发展水平地区间变异系数高达91.25%。平原地区的公共卫生安全水平综合贴近度最高(0.525 0),其次为海岛(0.353 9)和山区(0.333 3),后两者应急处置维度贴近度均接近理想值1.000 0。县级市的公共卫生安全水平综合贴近度最高(0.666 0),其次为市辖区(0.435 9),最后为县(0.382 2),其中健康环境和应急处置维度在3个不同行政类别地区间变异系数较高,分别达68.77%和51.93%。
    结论  浙江省不同特征县(市、区)的公共卫生安全水平存在一定的非均衡性。中等发达地区的总体公共卫生安全水平较高,以健康环境较为突出。山区海岛具有突出的应急处置优势,但健康环境和监测预警能力不及平原地区。县级市的总体公共卫生安全水平较高。

     

    Abstract:
    Objective  To understand the equilibrium status of public health security in different regions at the county (county-level city, district) level in Zhejiang province, thus providing a basis for targeted policy implementation across the entire province.
    Methods On the basis of the public health security evaluation index system, relevant public health security data in 2023 were collected from seven counties (county-level cities, districts) in Zhejiang province. The entropy weight technique for order preference by similarity to ideal solution (TOPSIS) was used to calculate the index values for each county (county-level city, district). The comprehensive closeness degrees for six dimensions including policy guarantee, resource support, healthy environment, surveillance and early warning, emergency response, and vulnerable population were calculated upon the grouping by different socioeconomic levels, different terrains, and different county-level administrative categories. The coefficients of variation were calculated to assess the level of balance in public health security development.
    Results The comprehensive closeness degree of public health security in moderately developed areas in Zhejiang was the highest (0.644 0), significantly higher than that in developed areas (0.399 3) and underdeveloped areas (0.344 6). The coefficient of variation for the health environment dimension among the three areas with different socioeconomic levels reached 91.25%. The comprehensive closeness degree of public health security levels in plains was the highest (0.525 0), followed by those in islands (0.353 9) and mountainous areas (0.333 3). The closeness degrees for the emergency response dimension in the latter two areas were both close to the ideal value of 1.000 0. County-level cities had the highest comprehensive closeness degree of public health security (0.666 0), followed by districts (0.435 9) and counties (0.382 2). The coefficients of variation for the health environment and emergency response dimensions among the three areas with different county-level administrative categories were high, reaching 68.77% and 51.93%, respectively.
    Conclusions The public health security levels in different counties (county-level cities, districts) of Zhejiang province exhibit a certain degree of non-equilibrium. In moderately developed areas, the overall public health security level is relatively high, with a particular emphasis on the healthy environment. The islands and mountainous areas have significant advantages in emergency response, while their healthy environment and surveillance and early warning capabilities are not as strong as those in plains. In county-level cities, the overall public health security level is relatively high.
