现场流行病学培训项目质量评估指标体系构建:基于德尔菲专家咨询法

Establishment of a quality evaluation index system for field epidemiology training program based on the Delphi method

    目的 构建一套适合“干中学”培训模式、培训周期较长的现场流行病学培训项目(FETP)质量评估指标体系,为有针对性地开展培训及提高培训效果提供测量工具。
    方法 以柯氏模型和CIPP模型为理论框架,采用文献查阅法和系统综述法初步确定FETP质量评估指标体系初步框架,遴选43名从事相关领域的专家开展2轮德尔菲专家咨询,根据合理性、重要性和可操作性评分进行指标筛选并计算各级指标权重。
    结果 2轮专家咨询的积极系数分别为93.02%和100.00%,专家权威系数分别为0.849和0.886。第1轮专家意见合理性、重要性和可操作性协调系数分别为0.118、0.127和0.089(均P<0.05);第2轮专家意见合理性、重要性和可操作性协调系数分别为0.179、0.189和0.163(均P<0.01),专家意见协调性较好。经专家咨询后,最终构建的FETP质量评估指标体系包括一级指标3个、二级指标8个和三级指标50个,三级指标合理性、重要性和可操作性变异系数(CV)分别为0.07~0.22、0.06~0.22和0.09~0.20;在构建的评估指标中,一级指标归一化权重为0.319~0.351,二级指标归一化权重为0.101~0.151,三级指标归一化权重为0.009~0.026。
    结论 本研究构建的FETP质量评价指标体系具有良好的科学性和可靠性,该指标体系在兼顾培训过程管理和效果评估的同时综合考虑了培训需求的重要性,为项目开展质量评估提供了测量内容和量化依据。

     

    Abstract:
    Objective To construct an evaluation index system for the Field Epidemiology Training Program (FETP) that adapts to the "learning while doing" training model and has a long training cycle, thereby providing a measurement tool for targeted training implementation and improvement of training effectiveness.
    Methods A preliminary quality evaluation index system for the FETP was established through literature review with the Kirkpatrick model and CIPP model as theoretical frameworks. Two rounds of Delphi expert consultation were conducted among 43 experts engaged in relevant fields. Indexes were screened based on scores for rationality, importance, and operability, and the weight of each index was calculated.
    Results The expert positive coefficient (100.00% vs. 93.02%) and expert authority coefficient (0.886 vs. 0.849) in the second round were higher than those in the first round. The concordance coefficients of rationality, importance, and operability of expert opinions were 0.118, 0.127, and 0.089 in the first round (P < 0.05) and 0.179, 0.189, and 0.163 in the second round (P < 0.01), respectively, indicating good concordance of expert opinions. After screening, the final index system for evaluation of FETP quality consisted of 3 first-level indexes, 8 second-level indexes, and 50 third-level indexes. The coefficients of variation (CV) of the third-level indexes for rationality, importance, and operability were 0.07–0.22, 0.06–0.22, and 0.09–0.20, respectively. The normalized weights of the first-level, second-level, and third-level indexes were within the ranges of 0.319–0.351, 0.101–0.151, and 0.009–0.026, respectively.
    Conclusions The constructed index system for the FETP quality evaluation demonstrates good scientificity and reliability. While balancing training process management and effectiveness evaluation, the system comprehensively considers the importance of training needs, providing measurement contents and quantitative basis for the quality assessment of program implementation.
