青岛市黄岛区2型糖尿病患者规律服药率和血糖控制率规范管理干预效果评价

Effectiveness of standardized management intervention on regular medication and glycemic control among type 2 diabetic patients in Huangdao district of Qingdao city

      目的  评价山东省青岛市黄岛区2型糖尿病患者规律服药率和血糖控制率的规范管理干预效果,为提高糖尿病患者血糖控制和防治水平提供参考依据。
      方法  于2012年5 — 7月整群抽取青岛市黄岛区15 369例 ≥ 35岁2型糖尿病患者建立糖尿病健康管理档案,进行为期3年的规范管理,给予免费糖尿病药物治疗且每年至少进行4次面访和空腹血糖测量,比较2型糖尿病患者干预前后的规律服药率和血糖控制率。
      结果  青岛市黄岛区2型糖尿病患者干预后的规律服药率和血糖控制率分别为51.99 %和47.61 %,均高于干预前的43.92 %和30.29 %(均P < 0.001);男性2型糖尿病患者干预后的规律服药率和血糖控制率分别为49.32 %和50.59 %,均高于干预前的38.81 %和31.74 %(均P < 0.001);女性2型糖尿病患者干预后的规律服药率和血糖控制率分别为53.60 %和45.84 %,均高于干预前的46.95 %和29.44 %(均P < 0.001)。
      结论  规范管理可提高该地区糖尿病患者的规律服药率和血糖控制率。

     

    Abstract:
      Objective  To evaluate the effect of standardized management intervention on regular medication and glycemic control among type 2 diabetes patients in Huangdao district of Qingdao city, Shandong province and to provide references for the improvement of blood glucose control and treatment in diabetic patients.
      Methods  We recruited all type 2 diabetes patients (n = 15 369) aged ≥ 35 years and established health care records for them from May to July, 2012. Then we conducted a 3-year standardized management program, including free of charge medication for diabetes and at least 4 times of both face-to-face follow-up and fasting blood glucose measurement per year, among the participants. The regular medication rate and blood glucose control rate before and after the intervention program among the participants were analyzed and compared.
      Results  Compared to those before the intervention, significantly increased regular medication rates (51.99% vs. 47.61% among all participants, 49.32% vs. 38.81% among male participants, and 53.60% vs. 46.95% among female participants) and glycemic control rates (47.61% vs. 30.29% among all participants, 50.59% vs. 31.74% among male participants, and 45.84% vs. 29.44% among female participants) were observed after the intervention (P < 0.001 for all).
      Conclusion  Standardized management intervention among type 2 diabetes patients could improve regular medication rate and glycemic control rate in community diabetes patients.
