环境铅暴露对儿童肾脏功能的影响

Effect on Renal Functions of Children Exposed to Environmental Lead

  • 摘要: 目的 了解环境铅暴露对儿童肾脏功能的影响.方法 根据血铅值将儿童分为4个组别,第1组血铅水平小于或等于0.483μmol/L;第2组血铅水平为大于0.483μmol/L小于或等于0.965μmol/L;第3组血铅水平为大于0.965μmol/L小于或等于2.172μmol/L,第4组血铅水平为大于2.172μmol/L.测定各组儿童的尿中NAG、γGT、ALP、LDH活性和尿总蛋白、β2MG、ALB以及IgG含量.结果 随着儿童血铅水平的增高,各种尿酶的活性也升高,尤以NAG和γGT活性升高幅度较大,NAG、LDH在血铅水平处于第3组时,已与第1组有显着性差异,说明NAG和LDH较为敏感.当血铅水平大于2.172μmol/L时,尿中β2MG含量显着性增加,而其它种类的尿蛋白均无明显变化.结论 环境铅暴露的儿童当血铅水平大于0.965μmol/L时,就已不同程度对肾脏造成损伤,当血铅水平大于2.172μmol/L时,病变更为广泛;NAG、LDH、γGT、ALP和β2MG可以作为对环境铅暴露儿童肾脏损伤的早期联合监测指标.

     

    Abstract: Objective To estimate the effect on renal function of children exposed to environmental lead.Methods Children were divided into four groups based on thier blood lead levels.The first group blood lead level were less than or equal to 0.483μmol/L;The second group blood lead level were more than 0.483μmol/L,or less than or equal to 0.965μmol/L;The third group blood lead level were more than 0.965μmol/L,less than or equal to 2.172μmol/L;The fouth group blood lead level were more than 2.172μmol/L.The activities of NAG,γ-GT,ALP,LDH and the content of total proteinβ2,-MG,Alb,IgG in the urine of different groups children were determinded.Results The higher of the blood lead levels of children,the higher of the activities of the enzymes in urine,especially NAG andγ2GT;The activities of NAG and LDH were significantly higher in the third group than those in the first group.When the blood lead levels were more than 2.172μmol/L,the contents ofβ2-MG in urine were significantly increased,and the contents of other kinds of protein in urine were not significantly changed.Conclusion When the blood lead levels were more than 0.965μmol/L,the renal functions of children exposed to environmental lead had been damaged in a certain degree,and when blood lead levels more than 2.172μmol/L,the damaging was more extensive;Determinitions of NAG,LDH,γ-GT,ALP andβ2-MG were regarded as early monitoring renal damaging index in children exposed to environmental lead.
