代谢综合征各组分对2型糖尿病预测价值

Predictive values of components of metabolic syndrome on type 2 diabetes mellitus

  • 摘要: 目的 探讨代谢综合征(MS)各组分对2型糖尿病(T2DM)的预测价值。方法 运用整群随机抽样的方法,对黑龙江省兰西县平山镇11个村屯35岁以上、≥5年的常住居民进行调查和体检,获取腰围(WC)、血压(BP)、甘油三酯(TG)、高密度脂蛋白(HDL)等数据,进行分析比较。结果 排除血糖后,MS单组分中,TG、WC对T2DM有影响(TG:χ2=18.19,P<0.0001;WC:χ2=10.25,P=0.0014),MS多组分中,WC合并(BP+TG)对T2DM影响较大(χ2=24.45,P<0.0001);交互作用分析表明,WC与MS其他组分两两串联或并联时均存在正交互作用(P<0.01,S>1),对T2DM具有增效作用;灵敏度、特异度分析表明,WC合并(BP+TG)对T2DM的灵敏度、特异度均较好,正确指数、人群归因危险度百分比(PARP)、阳性似然比(+LR)最高,诊断价值较高,对T2DM有较高的预测价值。结论 排除血糖后,MS单组分中TG、WC对T2DM有影响,是T2DM的预测因子,而在MS多组分中WC合并(BP+TG)时对T2DM的影响最大,预测价值最高。

     

    Abstract: Objective To analyze the predic tive values of the components of metabolic syndrome(MS)on type-diabetes mellitus(T-DM).Methods A cluster sampling method was used to collect local residents elder than 35 years in Lanxi county of Heilongjiang.Data on waist circum ference(WC),blood pressure(BP),triglyceride(TG)and high density lipoprote in(HDL)were obtained through investigation and physical check-ups and analyzed.Results MS,TG and WC were main in fluence factors of T-DM after excluded blood glucose(TG:χ2=18119,P<0.000 1;WC:χ2=10.24,P= 0.001 4).In multi-components of MS,the WC combined with(BP+TG)was a main influence factor of T-DM(χ2= 24.45,P<0.0001).The in teraction analysis showed that WC and other components of MS had positive interaction and synergistic effect on T-DM(P<0.01,S>1)with compounding in series or parallel.The analysis of sensitivity and specificity showed that in the components of MS,the sensitivity and specificity of WC combined with(BP+TG)on T-DM was better than other components with high correct index,PAR P,+LR.There fore,the components were of high predictive values on T-DM.Conclusion In the components of MS,TG and WC are main influence and predictive factors of T-DM after excluding blood glucose.In multi-components of MS,the WC combined with(BP+TG)is a main influence and predictive factor of T-DM.
