血清谷丙转氨酶与代谢综合征相关性

Correlation between serum alanine aminotransferase and metabolic syndrome

  • 摘要: 目的 探讨血清谷丙转氨酶(ALT)与代谢综合征(MS)及其组分之间的关系,为MS的预防控制提供参考依据。方法 采样分层整群抽样方法对在江苏省常州市武进区抽取的8 818名居民进行问卷调查、体格检查和实验室检测。结果 8 818名居民中,MS、中心性肥胖、血压升高、空腹血糖(FBG)升高、甘油三酯(TG)升高、高密度脂蛋白胆固醇(HDL-C)降低者分别1 907、3 264、4 674、2 369、2 843、1 591例,分别占21.62%、37.02%、53.00%、26.86%、32.24%、18.04%;ALT平均水平为(22.43±17.71)U/L,不同ALT四分位分组居民比较,随着ALT水平升高,MS、中心性肥胖、血压升高、FBG升高、TG升高、HDL-C降低者比例均呈增高趋势(均P<0.001);多因素logistic回归分析结果显示,在调整了性别、年龄、文化程度、吸烟、饮酒等因素后,随着ALT水平的升高,Q4组居民发生MS的风险为Q1组居民的5.69倍(OR=5.69,95%CI=4.72~6.86);进一步调整中心性肥胖、血压升高、FBG升高、TG升高、HDL-C降低5个MS组分后,Q4组居民发生中心性肥胖、血压升高、FBG升高、TG升高的风险分别为Q1组居民的3.20倍(OR=3.20,95%CI=2.74~3.75)、1.36倍(OR=1.36,95%CI=1.17~1.58)、1.87倍(OR=1.87,95%CI=1.58~2.20)、3.52倍(OR=3.52,95%CI=2.98~4.15)。结论 血清ALT水平与MS及其组分相关,血清ALT水平可能成为MS患病的一个预测指标。

     

    Abstract: Objective To investigate the relationship between serum alanine aminotransferase(ALT)and metabolic syndrome(MS)and its components.Methods A rural residents sample was selected in Wujin district of Changzhou city with multi-stage cluster sampling method for a study including questionnaire survey, physical examination and laboratory test.Results Among the 8 818 residents, the prevalence of MS, central obesity, elevated blood pressure, elevated fasting blood glucose(FBG), elevated triglycerides(TG), and decreased high-density lipoprotein cholesterol(HDL-C)were 21.62%, 37.02%, 53.00%, 26.86%, 32.24%, and 18.04%, respectively.The average ALT level was 22.43±17.71 U/L and the prevalence of MS, central obesity, elevated blood pressure, elevated FBG, elevated TG, decreased HDL-C were increased with ALT quartiles in the residents(P<0.001 for all).Multivariate logistic regression analyses showed that the risk of MS was increased with ALT quantile levels after adjusting for age, sex, smoking, alcohol consumption, and education level, with an odds ratio(OR)of Q4 group of 5.69(95%confidence interval95% CI=4.72-6.86)compared to Q1 group.The risk of central obesity, elevated blood pressure, elevated FBG, and elevated TG increased with ALT quantile level after adjusting for central obesity, elevated blood pressure, elevated FBG, elevated TG, decreased HDL-C, with the OR of 3.20(95%CI = 3.20-3.75), 1.36(95%CI = 1.36-1.58), 1.87(95% CI =1.87-2.20), and 3.52(95%CI = 3.52-4.15) for Q4 group compared to Q1 group.Conclusion Serum ALT level is associated with MS and its components and serum ALT level may be a predictor of MS.
