Relationship between dyslipidemia and diabetes mellitus among adult residents in Jiangsu province

摘要: 目的探讨江苏省成年人血脂异常与糖尿病的关系, 为制定血脂异常和糖尿病的综合防治策略和措施提供科学依据。方法采用多阶段整群随机抽样方法在江苏省14个成人慢性病及其危险因素监测点抽取8 400名≥18岁成年居民并进行面访问卷调查, 分析血脂异常与糖尿病的关系。结果江苏省成年人血脂异常患病率为35.0%, 其中总胆固醇(TC)升高率为4.7%, 高密度脂蛋白胆固醇(HDL-C)降低率为21.1%, 低密度脂蛋白胆固醇(LDL-C)升高率为1.7%, 甘油三酯(TG)升高率为13.4%;糖尿病患病率为8.5%;相关分析结果显示, TC、LDL-C、TG与空腹血糖均呈正相关(P=0.000), HDL-C与空腹血糖呈负相关(r=-0.054, P=0.000);多因素非条件logistic回归分析结果显示, 血脂异常、TC升高、HDL-C降低和TG升高的成年人糖尿病患病风险分别为正常成年人的2.40倍(OR=2.40, 95%CI=2.03~2.83)、1.91倍(OR=1.91, 95%CI=1.41~2.58)、1.77倍(OR=1.77, 95%CI=1.47~2.13)和2.24倍(OR=2.24, 95%CI=1.83~2.73);TC、LDL-C和TG每增加1个标准差, 糖尿病患病风险分别增加17%(OR=1.17, 95%CI=1.07~1.26)、20%(OR=1.20, 95%CI=1.11~1.30)和26%(OR=1.26, 95%CI=1.19~1.34), HDL-C每增加1个标准差, 糖尿病患病风险降低32%(OR=0.76, 95%CI=0.70~0.84), TC、LDL-C、TG和HDL-C对糖尿病患病风险的影响依次升高。结论江苏省成年人血脂异常与糖尿病密切相关, 应加强血脂异常与糖尿病的综合防治。Abstract: ObjectiveTo explore the relationship between dyslipidemia and diabetes mellitus in adult residents of Jiangsu province,and to provide a scientific basis for formulating strategies and measures for comprehensive prevention and control of dyslipidemia and diabetes mellitus.MethodsMulti-stratified clustering sampling method was used to sample 8 400 residents aged 18 years or above from 14 disease surveillance points of Jiangsu Provincial Surveillance on Chronic Diseases and Risk Factors in Adults.The data were collected with a face-to-face questionnaire survey and analyzed with SPSS 18.0.ResultsThe prevalence of dyslipidemia was 35.0%,with the prevalence rate of higher total cholesterol(TC)of 4.7%,lower high density lipoprotein cholesterol(HDL-C)of 21.1%,higher low density lipoprotein cholesterol(LDL-C)of 1.7%,and higher triglyceride(TG)of 13.4%.The prevalence rate of diabetes mellitus was 8.5%.The results of correlation analysis showed that fasting blood glucose had positive correlation with TC,LDL-C,and TG(P=0.000) and negative correlation with HDL-C(r=-0.054,P=0.000).The results of multivariate unconditioned logistic regression analysis showed that the risk of diabetes mellitus were 2.40,1.91,1.77,and 2.24 times higher for the adults with dyslipidemia,higher TC,lower HDL-C,and higher TG,respectively,compared to those of normal adults;and an increase of one standard deviation of TC,LDL-C and TG were associated with 17%(odds ratio[OR]=1.17,95% confidence interval[95%CI]=1.07-1.26),20%(OR=1.20,95%CI=1.11-1.30),and 26%(OR=1.26,95%CI=1.19-1.34)higher risk of diabetes mellitus,and an increase of one standard deviation of HDL-C was associated with 32%(OR=0.76,95%CI=0.70-0.84)of decreased risk of diabetes mellitus,respectively.The risk of diabetes mellitus increased in turn with the influence of TC,LDL-C,TG,and HDL-C.ConclusionDyslipidemia is closely associated with diabetes mellitus in adult residents of Jiangsu province.Comprehensive prevention and control of dyslipidemia and diabetes mellitus should be strengthened in the population.
